53 yo very healthy man with unfav int risk prostate cancer Gleason 4+3 in 12 of 16 targeted biopsy cores PSA 16 ng/ml (was 14 back in November) had a PSMA for staging that shows Suvmax uptake of 4.1 in a small perirectal node (see picture). Chemotherapy + Orgo 1?
Would love some advice. Been hearing horror stories about it my entire freshman year and I was unconfident of doing good in it before my cancer diagnosis. The chemotherapy is set to end September 3rd, and my orgo class starts early August. I looked at the syllabus and apparently I will be under chemo for effectively 25% of the course (the first exam is on September 15th). Bone met scenario
Here's a scenario I need help with: 70ish yo M with castrate-resistant stage IV prostate cancer. PSMA PET showed a mid femur met along with other small bone lesions, the patient was asymptomatic. I radiated the mid femur met 30 Gy in 10 fractions hoping to prevent a fracture (based on the data presented at ASTRO). PSA goes down (patient is on xtandi btw).
