Skin Squamous with brain invasion
Got a guy with multiple recurrences of skin cancer of the scalp who now has brain and bone invasion locally just adjacent to the precentral gyrus. 78yo and a KPS of 70. Anyone think more resection is necessary since they will not resect the area well enough to get a GTR? Should I just do 60 Gy and 30fx and tell neurosurgery to hold off? Can't really find much literature on outcomes once the brain parenchyma is involved by skin cancer. (Source: Student Doctor Network)

Thyroid Neck Recurrence
I got a guy that presented 2 years ago with metastatic thyroid cancer; they did a hemithyroidectomy at presentation to get more definitive diagnosis. Turned out to be hurthle cell ca. They did adjuvant RAI after this although the tumor wasn't hot for iodine uptake. There are 4 mets in the lungs that is it, they are about 1-2 cm and one is 2-3 cm. Prostate cancer IMRT - no difference between daily IGRT and weekly KV

Adjuvant Prostate Patient w Apparent Bulky Nodes
57 yo guy high anxiety diagnosed 3 months ago with very high risk prostate cancer Gleason 4+5 in all 12 cores PSA 10 ng/ml. Bone scan was negative/CTAP did not show any adenopathy. He did get referred beforehand to speak about RT. I basically told him look you're young but we dont have evidence of survival benefit with trimodality therapy, your chance of needing RT after surgery is essentially assured. Quick prostate question
Clinical staging of prostate cancer -- this may be a dumb question but I swear I have seen multiple ways used in my institution. Is clinical staging "overruled" by a DRE, biopsy, and/or MRI? In other words, let's say a guy had cT1c disease because it was all found on screening PSAs. - What if this guy had a DRE that showed a suspicious right sided nodule without ECE/SVI - do you still call it T1c or is it T2b? - What if this guy then had a biopsy and found to have prostate cancer... Why are bench research labs often such toxic work environments?
There are countless posts here about PI's, postdocs, and grad students abusing, manipulating, or otherwise having aggressive/inappropriate behavior towards pre meds and/or others below them. Gastric cancer local spread
Hey, FA states; "Early aggressive local spread with node/liver metastases" for gastric cancer. What does that mean exactly? Does this mean that for every type of gastric cancer, if metastasis to the liver or local nodes (e.g. sister Mary Joseph nodule but NOT Virschow node as that is distant) then the gastric cancer is early in progression but is aggressive. Let's say you are planning to retire before you get Medicare health insurance due to age and you want to get private insurance. Looking around the cheapest available for a couple are the high deductible at around $950 a month with $14,700 total deductible and no out of pocket costs afterwards.
