HOPD Employee- How busy?
Hi everyone, I am looking into an employed position at a community hospital. They do not currently have a pain department and I would be helping to establish it. I was told by the CEO that there would be no difficulty filling my clinic with spine pain and filling two days for procedures. I want to provide the best patient care, but at the same time I am willing to hustle and work hard. They told me they would do all the marketing for me (put me on billboards, TV, etc) in the community and... Read more (Source: Student Doctor Network)

Patient advice
I have a mid-60 year old patient with remote IV drug abuse, HIV and previous lumbar laminectomy still having low back and leg symptoms. Has failed all non-opioid meds, LESIs, MBBs, SI joints. Is currently doing a spinal cord stim trial but not getting a lot of relief and seems to be heading in the direction of a failed SCS trial. At last visit leading up to the trial, patient’s partner put a lot of pressure on me to prescribe an opioid and that it is terrible to have to see patient deal with... Doing Pain or Anesthesia?
With inflation, reimbursement cuts, and staffing costs since pandemic has anyone practicing considered going back to Anesthesia? Some of these rates are undeniably great. I'm still young, when I decided to do my pain fellowship the market was hot for outpatient pain jobs, now I see a trend for Anesthesia jobs, there is a definite shift happening and I am wondering if many people who are in pain are considering switching back? And if you are not switching back is it because you haven't done... Read more (Source: Student Doctor Network)

How to improve spine MRI reads
Hi all, About to start my pain fellowship and one of my glaring deficiencies (among many) I've realized is independence in reading spine MRIs. Our residency had very good exposure to pain, but I feel like I didn't make as much of an effort to become proficient at this so I wouldn't have to rely on a radiologist's interpretation. A lot of the time my attending would point out abnormalities that I either could or could not appreciate, and then I didn't necessarily think about it again. Does... RFA pain
Odd case yesterday and I'm hoping to get some hive mind input. Did an RFA on a patient and during the ablation around 70 degrees Celsius mark during ramp up the patient had significant axial low back pain. Prior to the burn Took AP,15,30, and lateral imaging and everything looked great. Motor testing negative too. Gave some additional lido 2% and restarted with repeat pain. Repeat imaging good, aspiration neg, and motor stim negative again. Eventually I wound up removing two probes and... Read more (Source: Student Doctor Network)

Sports and Pain
Hey everyone, if this has been answered in the past definitely let me know and I can delete. I ultimately want to do Sports Med with my career but understand that it’s hard to swing a fully sports practice so people choose another area to fill the gap ie inpatient rehab, snf, MSK stuff. If I wanted to alternate with sports med and do spine procedures on the other days is this technically what the Sports and Spine fellowship is meant for? Is there a reason I shouldn’t be considering this? As... Read more (Source: Student Doctor Network)

Advice switching back to anesthesiology from pain medicine
Hello. I was wondering if anyone can give me some advice on switching back to anesthesia from pain medicine. I graduated from fellowship last year and have been practicing 100% pain for almost 2 years. I was thinking of switching back but really nervous about making the switch and I feel stuck. I am board certified but feel that my anesthesia skills have severely diminished. I was thinking of asking new jobs to see if any anesthesiologists were willing to shadow me until I get back into... I want to do ESI under her MVA ( she has Med Pay $10,000 ) but my billing wants to do her under original commercial insurance as she said if she is exhausted with MedPay , I will not be able to treat her and we can not bill her regular insurance after we go through her MVA!
