OMM/OMT for AOBPMR Written Board
For those who have recently taken the AOBPMR written board, how much OMM/OMT was involved? Was it similar to having to study broad scope of OMM/OMT for COMLEX? I didn’t even think about it until now and realized because it’s the DO board it may have OMM questions, so I checked their website which groups OMM with pain and spine under the category of interventional therapeutics for 25-39% of the exam. What does that even mean :/ I’m wondering if I need to dive into the green Saverese OMT... Read more (Source: Student Doctor Network)

Repeat Surgery vs SCS with post surgical foraminal stenosis
I see this a lot Someone has had a back surgery, still has radicular pain Formaminal stenosis on MRI (moderate or severe lets say), at a different level Does not want anymore back surgery, period. No weakness Has failed injections, antineuropathic meds. I typically do not offer SCS. It just seems like the indicated procedure is a nerve decompression, so that is what they should get, not an SCS which is just going to mask the pain. And I dont even know how good SCS is at masking pain... Read more (Source: Student Doctor Network)

Lead apron suggestions?
I’m shopping for some new lead aprons as our current ones are pretty worn out. I do not like wearing the two pieces vest and skirt design (annoying to remove and put back on) and prefers the one piece design. Also the old ones are super heavy and I could use something lighter. Any recommendations on lead apron vendors for something lightweight with good ergonomics and shielding? What’s the minimum lead equivalent I should aim for without killing my back? Does advanced imaging guide therapy
I’m a PMR/spine specialist and was curious if MRIs help you all when treating chronic neck and/ or low back pain? If so can you give specific examples of how it would guide your treatment plan in both the neck and low back. Appreciate all that you do (Source: Student Doctor Network)

Options for Pancoast tumor pain
One of the local family docs called me - his 100 year old patient was having arm pain and Horner's syndrome, and lo and behold we found a huge Pancoast tumor invading into brachial plexus, with bony mets in the cervical spine as well. He's sending her to hospice, but wants to know if I have any palliative procedures I can do first. Would any of you try a neurolysis? Anatomy seems too messy to try a brachial plexus catheter. Any other ideas? Should I just let hospice do what they can with... Read more (Source: Student Doctor Network)

Non-healing wound over an IT pump
I'm looking for some advice on how to handle this case. I have a 76 year old male with a history of numerous low back surgeries. He is fused from L3-S1 and has conservative therapies. I implanted a pump in March. His 10 day post-op visit was unremarkable. 2 weeks after the implant, he fell, sliding off of his bed onto the floor and sustained abrasion to the skin over his pump, well below the incision line. I saw him back for a recheck in early June and he had developed a remarkable... Read more (Source: Student Doctor Network)

Lumbar RFA after fusion
Patient has L3-L5 posterior fusion and R SI fusion. Complains of persistent low back/buttock pain worse to the right side. No extremity pain, just some intermittent n/t to lateral feet. Back pain worse with standing, no pain while seated. On exam, +facet loading in low back but nothing above fusion site. Tenderness over right SI. Would you do an L5 DR block by itself? Would insurance pay for this?? (Source: Student Doctor Network)

Pain clinic chain to pay $11.4M to settle Medicare and Medicaid fraud claims
“ The settlement also alleges Lagattuta encouraged unsuitable patients to undergo spinal cord stimulation. It describes the procedure as “an invasive surgery of last resort,” in which implants placed near the spinal cord apply low-voltage electrical pulses to nerve fibers. Lagattuta paid a psychiatrist $3,000 each month to falsely certify that every Lags Medical candidate for the procedure had no... Read more (Source: Student Doctor Network)

What can I do to get into a top PM & R program?
Hi. I'm a MS3 in a mid-tier US MD school with a home PM&R program. I just started clerkships several months ago and I have high passed surgery and psych. As far as I'm aware, I do not have any red flags other than being an average/below average student during pre-clinicals. My other experiences include: Research: formerly in a regenerative medicine lab but project was abruptly halted. Currently submitted a case report for a pain/interventional spine projects and hopefully also submitting to... Read more (Source: Student Doctor Network)
