Northern California Private Practice Ortho Group
My group is looking to hire another interventional pain doc due to volume and community need. I’m anesthesia/pain and do mostly bread and butter injections, lots of RFA, some joint injections, and some SCS, and my partner who is PM&R/spine fellowship does lumbar interventions and EMGs. Procedures almost all done in clinic including SCS trial and kypho. No opioid management (unless you want to). I don’t. Partner does a tiny bit for long-time patients. 2 local long-standing pain Docs retired... Read more (Source: Student Doctor Network)

Cochrane review concludes SCS is ineffective for chronic low back pain
https://www.cochranelibrary.com/cdsr/doi/10.1002/14651858.CD014789.pub2/full “Authors' conclusions Data in this review do not support the use of SCS to manage low back pain outside a clinical trial. Current evidence suggests SCS probably does not have sustained clinical benefits that would outweigh the costs and risks of this surgical intervention.” (Source: Student Doctor Network)

C7-1 vs C6-7 CESI
Hello everyone, I see some people doing C6-7 ILESI's and these physicians swear they get better relief. I know the ligament things as we move further superior but they say C6-7 is still ok. I saw a patient I did 2 C7-1 ESI's and she got a third from another at C6-7 with 100% relief as opposed to only moderate relief with mine. Are other people doing C6-7? I know in theory the epidural space is small enough that volume does get up the spine but wanted to see what others are doing. Thank yall! (Source: Student Doctor Network)

Bizarre practices
I’ve had a lot of patients leaving their pain doctors in the last year to come see me after failed treatment. First practice is a well known and respected one in my community. Interesting enough, many of his patients had a RACZ procedure. What was more interesting is none of them had back surgery or other procedure prior to his injections. After further testing, none had MRI findings of epidural scaring or nerve clumping. After talking to many of them it sounds like he uses the Racz... Read more (Source: Student Doctor Network)

Extrapedicular Vertebral Augmentation
First time I have reason to do a true extrapedicular kypho. 60 y/o M on chronic glucocorticoids presents with severe low back pain. Edema noted at L3 and L4, patient with prior instrumented fusion at L4-5. Screws are fairly superior at L4, so I was planning on using Beall's extrapedicular inferior end plate access, aka low and outside (3rd picture). Since I hadn't augmented a fused level before, Stryker connected me to one of their KOLs. He was... Read more (Source: Student Doctor Network)

Sedation protocols for RFA/rhizotomy
In med school/residency, 5-10 years ago we did these with a wiff of versed and copious local in the office. New pain guys want essentially a motionless field, GA without an airway/ deep heavy propofol sedation in the OR. Sometimes they are just too fat or sick and there is no way to make this happen. Couple weeks back had a BMI 50 anxious chronic pain patient for bilateral c3-5 RFA. Usually I’d just tell pain guy I’m going to give a wiff of versed and deal with it. they expect more... What are your top 5 "Dirty Diaper" pain topics of 2022?
1) Opioid Prescribing Guidelines, COI, and Regulatory Capture of Federal Agencies. 2) Private Equity and Nonprofit Hospitals Buying Doctors: They will never love you back 3) Declining Medicare Reimbursement for Pain: Who cares, old people get what they deserve, and the payment is all in the facility fee anyway. 4) I'm a KOL but don't hate me because I'm beautiful. 5) I didn't even go to medical school, but anything you can do, I can do better.
