National Spine and Pain Centers
any new information on this group. National Pain and Spine Centers. Was interested in a job at practice owned by them. Was turned off by 3 weeks of vacation and 1 week of Educational leave. But wanted to know if any one had any new information on this big national group. (Source: Student Doctor Network)

pain mgmt doing microdiscectomies
I'm an M2 and I've heard from some anesthesia pain mgmt friends that they do micro discectomies and some other simple spine procedures like spinal cord stimulators. Are these the same type of procedures that neurosurgeons and orthopedic spine surgeons perform? (Source: Student Doctor Network)

Neurology Fellowships for an academic career in neuromodulation?
I’m interested in this broad field and was wondering which fellowships would be good for it. I know of a person who did pain fellowships and now has a good setup at a large-ish hospital, does a lot of spinal cord stimulation but his patient population seems to be relatively….. umm not my cup of tea. Mostly chronic pain with little improvement. I would personally like the same setup- a fair bit of procedure, opportunity for research but with more than just chronic pain patients. I’ve heard... OMT rant
Oh my goodness. OMT is absolute quackery. Maybe 1-2% of the techniques are worthwhile, but overall a complete and utter joke. We would do ourselves a favor as a profession if we reduced the nonsense. I don’t need to go poking my partners pubic symphysis to figure out that it is tender. Of course it hurts when you poke a finger above their genitals. Oh and lifting their thigh will cure their pain with a “counterstrain technique.” Sure it feels good on some areas of the back, so let’s simplify... Read more (Source: Student Doctor Network)

Private vs Hospital Employed
Hi there everyone, I learned last week that I will be joining your ranks as a pain doc as long as I don't get kicked out of my residency, and as long as I complete the fellowship I matched at. A little while back I had a fantastic job offer for a pain job after completing fellowship, however unfortunately my wife and I decided that the location was a deal-breaker (after Hurricane Ida) and I value her more than the $$$. My question to you all: Can anyone comment on what earnings as a... Read more (Source: Student Doctor Network)

new facet guidelines
What are the new facet joint guidelines that keep being referenced? I saw something a while back regarding limiting the numbers of facet procedure per rolling twelve months, but are there new medicare rules regarding therapeutic IA facet injections vs diagnostic? Is this just medicare for now or have some of the national carriers also changed their facet guidelines this year? (Source: Student Doctor Network)

Acute back pain in patient with SCS
54 year old with history of leukemia in remission. Left foot crush injury with CRPS. Had SCS placed several years ago. I did the trial and an SCS experienced NS did the implant. Right lead for non-specific LBP and left lead for the foot. Has done well. Last week complained of acute onset intense midline low back pressure. On exam only thing notable is TTP in midline over the horizontally placed anchor. Anchor does appear to be fairly superficial He has intentionally lost 75lbs since... SI fusion
I have a neurosurgeon near me that has been sending me a lot of diagnostic si injections for possible fusion recently. Up until now I’ve not been a big fan of the concept of fusing the si joint. I’m wondering if I should be a team player, do the shot, and send them back, or talk with the patient and give my honest opinion about si fusion. Wondering what everyone’s experience is with this, and if they’ve seen good outcomes from surgical si fusions (Source: Student Doctor Network)

Lost battle with the SIJ
A lost battle with my arch nemesis the SI joint today. Very petite 82 year old with osteoporosis s/p L4-S1 PLIF with transitional anatomy manifesting as lumbarized S1. Old fashioned right side iliac crest bone graft harvest scar. Right buttock pain. Weight bearing LS spine x-ray showing right SIJ and axial MRI image showing only cross sectional view of SIJ. On fluoro straight AP there was what looked to be a long radiolucent “stripe”. The remainder of the sacrum was just a gray ground glass... Read more (Source: Student Doctor Network)

Cervical RFA - approach
I recently started out of fellowship in hospital group. The rad tech is not best at optimizing lateral view for my cervical RFA (she just started as well). My approach in fellowship was as follows assuming L C4, C5, C6 MBNRF: 1) Get an AP shot of C-spine, do local anesthetic skin infiltration at lateral waist and drive one RF cannulae down (@C4) 2) Get lateral view, drive in lateral the other two cannulae to C5 and C6 (approximate medio-lateral position based on first cannulae at skin - be...
