FM doing pain procedures??
I had an Attending in residency Who would do a lot of epidurals injections (he had a fluoroscopy lab ) in addition to shoulders, knee, hip injections. What are your opinions on FM docs doing spinal pain procedures like epidural injections, facet injections… Etc. of course after training and certifications (not fellowship)? Referral Question
Long time lurker, first time poster. I am one year out of fellowship and working in a single specialty group. I am considering starting a practice from scratch in an area with a population of about 250,000. There are two other practices each with only one pain doc. There is a large Ortho group with two pain docs. There are two hospitals with 2 pain docs each. In regards to referrals, I know having a relationship with Ortho/spine surgeons is important. One issue I see is that most, if not... Read more (Source: Student Doctor Network)

Being a dentist is awesome!!
Hello, future dentists out there, I have been practicing dentistry since 2012. Let me tell you this: No regrets. The first five years of practicing is hard, but I have so much fun learning and making a real difference in people's life. Now I am going back for endo. But general dentistry has served me and my family very well. It is a good profession!!! Well, there are some days you will feel like sh***, but to get the patient out of pain, either endo or EXT third molars indeed make a... Read more (Source: Student Doctor Network)

What would you do?
I have an 80 year old woman with osteoporosis and prior compression fractures. She developed significant acute axial low back pain February 18th without any inciting event. This was so severe that it resulted in a short hospitalization for pain control. Workup was negative except for chronic L3 and L4 compression fractures (I have included sagittal STIR and T2 lumbar spine MRI images below from 2/19/2023). Patient was referred to me in March. Pain was facetogenic on examination. I ended up... Read more (Source: Student Doctor Network)

How dumb would it be to have surgery before Step 1 and surgery rotations
I have a partial tricep tear that needs to be repaired. It is scheduled for 6/19, with my Step 1 scheduled for 6/22, and surgery rotation scheduled for 7/3. I figure I'll be out of my brace by 7/3, and the surgery itself won't take that long so I'll really only lose 1 day. I figure the post-surgery pain will be gone by 6/22, considering I felt pretty much back to normal after a surgery 2 years ago within about a day or 2. For anyone who has had a triceps repair surgery, how dumb would this... Read more (Source: Student Doctor Network)

Back pain management
I'm almost 2 years out of training, still have some doubts regarding managing nontraumatic lower back pain in the ER: Obviously in people with no red flags, treat symptomatically and discharge. The main problem are these patient who usually have some worsening of their chronic back pain, coming in with some mild neuro complaint like one leg is starting to feel numb or a little weak, or some numbness in the groin. Assuming infectious workup is negative and they get a MRI non-con, always... Read more (Source: Student Doctor Network)

XRT for spine arthritis, or just extremities?
We've discussed many times here the merits of low dose RT for arthritis, but it seems to always be extremity. I guess my in-house program has taken off too well, because I got a referral for refractory spine arthritis pain, in a patient requiring interventions like nerve ablations. Has anyone here seen published or treated spine before? I'm sure someone must have tried it for ankylosing spondylitis or "rheumatism" back in the day. (Source: Student Doctor Network)

New Study Shows Chronic Pain is Real After all...
Has Science Cracked the Code on Chronic Pain? In a first, researchers have identified chronic pain pathways in individuals directly from their neural activity, showing promising paths forward for treatment. www.ucsf.edu If I were Deyo, Chou, anyone who ever published an article in the Back Letter, or therapeutic nihilists/pain deniers, I'd be pissed... Adding Fluoro Suite in Office - pay structure question
Hi All, I'm a PM&R Sports Medicine physician who did a fair amount of spine training in residency and fellowship. I work in an small (~8 doctor) ortho/sports group, mostly primary care sports, 2 surgeons. I predominantly do sports med, but am about 50% spine, doing ESIs and RFA (axial and joint). Up to this point I have been doing this in a hospital or ASC setting, as I added this to our practice and no spine care was established and we had no C-arm. Two years in we are looking to develop...
