Pain in neck and shoulder after posterior fusion
You guys know the patient has had posterior fusion. Lots of scarring in the center of the spine. TPI dont work or only transient. SCS is not an option d/t epidural scar likely. Therapeutic Disc injections for axial back pain

Holy crap! I did anesthesia today in a surgicenter for two neurosurgeons who did a bunch of spinal injections. I was blown away. One of the guys did two level bilateral transforaminal epidural steroid injections, all with an AP projection, no lateral projection - and 4 blobograms. Only one spinal level for RFA with commercial insurance?
Im in PA, and some pts have UPMC insurance. I have been getting denials for 2 level MBB and RFA by UPMC insurance and have looked into the policy and procedure manual for UPMC commercial insurance (again NOT medicaid), but insurance small business owners actually pay for. Two things obviously jump out 1. No more than 1 level can be ablated in 12 months 2. Cannot ablate at level with spondylolisthesis. HAHAHAHA this is absolutely laughable. Deciding between two jobs
I've been going back and forth between these two jobs. So thought I'd post this here and maybe get some insight. Job 1 (current job): -Private group practice -$420k salary (note: I'm in a no income tax state) - no bonuses of any kind, just flat salary. -7 hour work days -See ~25 patients on clinic days, probably do ~40 fluoro procedures a week in office. RFA doesn't last
So I'm sure this has happened to others before. Patient had 85% relief from bilateral lumbar RFA x2 months performed unilaterally 2 weeks apart. Pain then back to baseline. Exam consistent (facet loading +, same area, etc). What do you do? LCD guidelines state only 2 RFA's per 12 months. This is a Medicare patient so no prior auth required. I am hospital employed. MRI with nerve sheath thickening - anyone seen this before?
50+yo with new axial only low back pain of several months. Born in Southeast Asian country. I treated for facetogenic pain. Did MBBs and had ~70% relief but self-report limited by language/cultural barrier. In any case, pt decided to not move forward with RFA since was still "doing good". contract- APP revenue percentage
