60 yo M pmh throat cancer, diabetes, hypertension, hyperlipidemia, hypothyroid with worsening neck pain and unilateral arm weakness. Imaging shows atlantoaxial instability. Medial branch blocks/RFA after fusion
Patient has had prior L4-5 posterior decompression and fusion. Patient now has axial low back pain reproduced by facet loading bilaterally, has failed conservative measures including physical therapy, and MRI shows significant facet degeneration at L2-3, L3-4, and L5-S1 joints bilaterally. Anyone working four days a week?
I'm thinking about cutting back my workload to 4 days a week and hiring someone part time 4 days a week (or full time 5 if that's their bag). How does your staff handle things when you're out of the office one day a week? Are you expected to still be reachable during business hours? Even taking two half days off would be amazing. Hey, since fellowship I've been doing a lot of bread and butter and SCS. I'm trained in Vertiflex and mild. Stock market opinions?
Any opinions? I went in heavy in March and April. I did okay and I'm exiting now. I don't think a crash is imminent and I'm sure it will still go up for the next few weeks at least. There seems to be a lot of stimulus money being kicked around and I'm sure it will fuel the market but really, how long can this continue? I'm not sure but I don't want to stick around to find out. DLT and thoracic spine surgery
So we have a new surgeon who is starting to request lung isolation during thoracic spine surgeries. He’s primarily requesting it for big thoracic fusions and recently a corpectomy/fusion for epidural abscess. I remember this request a couple times in training, but it was rare. Abdominal pain in women
Alright, 6 months out and this one keeps getting me extended LOS and honestly when I get 2 or 3 at a time it seems like my cognitive load capacity is just depleted. Had two tonight. Young women, present in agonizing pain. Maybe a little tacky, maybe not. Tender belly. Improve with morphine. CT neg. Occupational hand radiation
I am admittedly careless at times with my hands in the path of the x-ray during fluoroscopic procedures. I am otherwise careful with radiation safety, wearing lead glasses, taking a step back during laterals, generally avoiding live fluoro, etc. I was led to believe that incidental exposure to the hands for the procedures we do was generally benign. Help with SCS lead location
Need some ideas. I have a patient that has a history of rectal pain/tailbone pain. 77 year old male, history of L4-L5 laminectomy 15 years ago - essentially not complaining of much back pain. Had some urological procedure (i think prostate biopsy) successfully, but then started having some discomfort in tailbone area a couple of months later. RLS > spinal
Did a total knee earlier today on a ASA 3 lady with restless leg syndrome, easy uneventful spinal. But patient is jerking her legs nearly the entirety of the case. She's snoring away the whole case and has zero pain post-op so the spinal obviously worked. How the hell is she moving her legs though? Anyone experience anything like this and if so any tricks? Chronic testicular pain
