Switching from efavirenz to rilpivirine improves sleep quality and self-perceived cognition but has no impact on neurocognitive performances
Conclusion: Our results do not support the hypothesis that switching to RPV improves cognitive function in patient under stable treatment with EFV. Nonetheless, improvements in neuropsychiatric symptoms, sleep quality and self-perceived cognition were observed. (Source: AIDS)

Effects of Highly Active Antiretroviral Therapy on Albuminuria in HIV-infected Persons.
CONCLUSIONS: HIV induces renal impairment. Thus, albuminuria, microalbuminuria and nonalbuminuric renal insufficiency are highly prevalent in antiretroviral-naïve HIV-infected persons but nephrotic-range albuminuria is uncommon. Albuminuria and/microalbuminuria and eGFR improve concurrently on HAART (with/without tenofovir DF). Zidovudine-based HAART (ZDV/3TC/NVP) resolves microalbuminuria earlier, and without relapse, unlike Tenovofir-based regimen and zidovudine with efavirenz (ZDV/3TC/EFV). Effects of Long-term Exposure to Tenofovir Disoproxil Fumarate-Containing Antiretroviral Therapy on Renal Function in HIV-Positive Chinese Patients
ConclusionsTDF-related renal impairment remained rare in HIV-positive Chinese patients with a median age of 29 years who had no comorbidities. A lower weight and duration of ART were associated with decreased renal function. (Source: Journal of Microbiology, Immunology and Infection)

Assessing the lymphoid tissue bioavailability of antiretrovirals in human primary lymphoid endothelial cells and in mice
ConclusionsARVs displayed distinct HLEC penetration patterns. PK studies of representative ARVs in LT of mice were concordant with HLEC bioavailability. These findings support further development of this approach and its translational predictability in humans. (Source: Journal of Antimicrobial Chemotherapy)

Switching at Low HIV-1 RNA into Fixed Dose Combinations: TDF/FTC/RPV is non-inferior to TDF/FTC/EFV in first-line suppressed patients living with HIV
Conclusion: In adults with suppressed viral load on first-line NNRTI-based ART in LMICs, switching to an STR of TDF/FTC/RPV was non-inferior to TDF/FTC/EFV in maintaining high rates of viral suppression with a comparable tolerability profile. (Source: Southern African Journal of HIV Medicine)

Nature and prevalence of adverse drug reaction of antiretroviral medications in Halibet National Referral Hospital: a retrospective study.
CONCLUSION: ADRs associated with ART drugs in Halibet hospital were found to be highly prevalent. Furthermore, CD4 count below 200, was identified as a major risk factor that predisposes patients to ADRs. This is burdensome to resource constrained countries such as Eritrea who have limited drug options and high HIV prevalence, therefore these findings will help patients and healthcare professionals understand the nature as well as seriousness of these ADRs and identify the risks involved with ART medications which can help minimize ART associated ADRs early on. Effects on immune system and viral reservoir of a short-cycle antiretroviral therapy in virologically suppressed HIV-positive patients
Conclusion: No significant immunological or viral changes were induced in the 3W group confirming the virological efficacy and immunogical safety of this strategy. In-depth virological and immunological analyses are useful in providing additional information in antiretroviral switching studies (Clinical Trials.gov: NCT01778413). (Source: AIDS)

Randomized clinical trial on efficacy of fixed-dose efavirenz/tenofovir/emtricitabine on alternate days versus continuous treatment
The objective of this study was to investigate the noninferiority of the TDF/FTC/EFV fixed-dose combination on alternate-days versus standard regimen in virologically suppressed patients. Design: A randomized-controlled open-label noninferiority trial enrolling HIV-1-infected patients treated for at least 6 months with TDF/FTC/EFV fixed-dose combination, virologically suppressed ( (Source: AIDS)

Race/Ethnicity and Protease Inhibitor Use Influence Plasma Tenofovir Exposure in Adults Living with HIV-1 in AIDS Clinical Trials Group Study A5202.
Conclusion Race/ethnicity is associated with tenofovir oral CL in HIV-1 positive, treatment-naïve adults. This covariate relationship raises questions about the possibility of differences in efficacy and risk of adverse events in different patient populations, and suggests that examining pre-exposure prophylaxis regimens and TFV exposure in different race/ethnicity groups be considered. Symfi, Symfi Lo, and Cimduo for HIV (online only)
Date: January 14, 2019 Issue #:  1563Summary:  The FDA has approved three new once-daily fixed-dose antiretroviral drug combinations for treatment of HIV-1 infection.Symfi andSymfi Lo (Mylan) contain the non-nucleoside reverse transcriptase inhibitor (NNRTI) efavirenz and the nucleoside reverse transcriptase inhibitors (NRTIs) lamivudine and tenofovir disoproxil fumarate (TDF) and were approved as complete antiretroviral regimens.Symfi Lo contains a lower dose of efavirenz than bothSymfi andAtripla, a fixed-dose combination of efavirenz, TDF, and the NRTI emtricitabine that was app...

Objective and Subjective Improvement of Cognition After Discontinuing Efavirenz in Asymptomatic Patients: A Randomized Controlled Trial
Conclusions: Switching from Atripla to Eviplera resulted in objective cognitive improvement on the group level in cognitively asymptomatic patients. Discrepancies in objective and subjective cognitive complaints make it challenging to identify patients who would benefit from discontinuing efavirenz. Pharmacokinetic (PK) interaction of fixed-dose combination (FDC) antiretrovirals (ARV) with riociguat (rio)
Conclusion: Rio exposure, when co-administered with ARV regimens, ranged from no apparent change with ATR to an approx. 3-fold increase with TMQ. In pts on stable FDC ARVs rio should be initiated at 0.5mg tid and pts monitored for signs and symptoms of hypotension at initiation and on treatment. (Source: European Respiratory Journal)
