Contrast Allergy and SI joint injection
I have a patient with a true allergy to contrast. I'm planning to perform an SI joint injection for her. What would you guys do: a) Do injection without contrast and utilize AP / lateral views with fluoro for needle confirmation? b) Have her undergo pre-treatment for a contrast allergy prior to injection and then perform injection c) Other My gut say to just do it without contrast but I am interested in others thoughts/ experiences. Failed Step 2 CS? Please help!!
Hey all, just wanted to see what you guys thought of the mistakes I made during the exam. In one encounter, pt told said he had an allergy to something but in my haste on the note, wrote that he had none... In another patient I wrote that lungs were clear bilaterally in haste and bc of carelessness but when I was listening to lungs, the exam was hindered by a particular sign/symptom. Here's my story. I have BS in Biology and entered with the intention of becoming a doctor. A few unfortunate personal issues resulted in a checkered undergrad transcript with 2.8 GPA, 3.2 in the sciences. I got discouraged and put off med school, but know that this is what I want now. I have a year of clinical experience working as a clinical allergy technician and loads of lab and research experience (although both were quite awhile ago, but still post UG). Sir, do you think it could be allergies?
"I've got the sinus (yes, singular) again doc!" Why is the suggestion that this "sinus infection" is really just allergies almost fightin' words? My area is notoriously bad for seasonal allergies but to ask most of my patients to try allergy medicine feels like I'm selling ice cubes to an Eskimo. Mr X, I've seen you every May for the last 5 years for this same thing. You got a Zpack from the immediate care 3 weeks ago and are still no better. Have you tried Zyrtec, Claritin, Flonase, etc?... Self-prescribing
I'm starting my pgy1 year in July, and out of curiosity, I was wondering what the legalities / ethics is behind prescribing non-controlled substances to oneself. Ie. Flonase, or Claritin. Obviously not any super serious meds, and definitely not Benzos or opioids. Sorry for my ignorance; I know it's probably frowned upon, but prescribing allergy meds for myself seems very benign. Interesting facts - related to veterinary med
I learned from a veterinarian that Collies have a high allergy rate for a majority of medicines especially if they have a specific gene mdr1. This occurs in 75% of both rough and smooth collies. If your dog has this they cannot pump specific drugs out of their brain and cause neurological defects and can result in death. What interesting facts have you learned?
