Peri-operative Antibiotics question
Hello all, I have a question I'm not sure about the answer or where to post on the forum. Patient is going to have knee replacement and has penicillin allergy, although he is stable on cephalexin. What would be the best antibiotic to use peri-operatively? Choice of antibiotics: cefazolin, vancomycin, clindamycin. I was thinking cefazolin because it's a 1st gen like cephalexin, but a friend argued for vancomycin. What do you guys think? (Source: Student Doctor Network)

Sulfa allergy -Meloxicam
Hey guys, had a patient with sulfa/sulfonamides allergy who I dispensed Meloxicam. Pt states that sulfa is in meloxicam structure so she is having anaphylactic reaction due to taking Meloxicam 8 hours earlier I checked Lexicom/Clinical pharmacology, and I just cant find it being as contraindication... Of course, pt is threatening to go to a hospital/sue/etc... Am I overlooking it as a contraindication? Can you guys double check me? (Source: Student Doctor Network)

Tape / adhesive allergy
I have a patient interested in an SCS trial but has a severe allergy to tape/adhesives including paper tape. When she had back surgery about three years ago, the adhesive from the dressing caused her significant dermatitis that still persists along her low back. Pt. Has twitching eye after NSRCT #12
Hi everyone, D3 here. I completed RCT on #12 a couple weeks ago, and my pt. has a CC two weeks later of “his eye is twitching” on the ipsilateral side of injection and his eye has not stopped since the day of the procedure Pt. Reports no allergies, med hx is remarkable, denies social use of alcohol, drugs and tobacco. BP is in elevated range and has no hx of surgeries. Hi everyone, I'm a pre-med on the interview trail. I'm looking to practice an Outpatient based specialty, like primary care or an IM based specialty like allergy and immunology. What could time off as a doctor look like? I want to be able to take a couple of weeks in blocks so that I can visit my relatives overseas. As well as take time off during Ramadan, or even just take a lighter load of patients for that time. Is it doable? I think I would go into private practice, but I would like... COMLEX Scores for Allergy/Immuno and Rheum
Hi. Currently 2nd year in Osteopathic med school. Interested in Allergy/Immuno and Rheum. I am aware that this is fellowship after IM. Wanted to know how competitive it will be for a DO student. I do not plan to take USMLE, just COMLEX since I will have to do IM to reach specialization. Does anyone else find it crazy how most specialties are stratified?
So hear me out. While doing an analysis of all the specialties, I’ve come to a conclusion. It seems like all the “cool” specialties have really long training times and long hours in addition to being super competitive (surgical specialties, IR, and other procedure-heavy fields), the ones that allow you to make ridiculous money but be bored at work are really competitive (derm, rads/radonc, allergy, optho), and then once people find out about how much you can make in a “less competitive”... MBA during fellowship?
Hello all, I'm a med peds resident who will begin an Allergy/Immunology fellowship next year (location TBD). I have grown more and more interested in health systems and the role of technology in patient care. I'd ultimately like to end up in a leadership role in my division or within a large health center.
