A Case of Hypercalcemia from PTHrP-Producing Fibromyxoid Sarcoma Responsive to Glucocorticoid Therapy
We report a case of PTHrP-induced hypercalcemia due to a malignancy —low grade fibromyxoid sarcoma—that responded to glucocorticoid treatment. This is the first report of glucocorticoids controlling PTHrP-mediated hypercalcemia of malignancy. Approach to the differential diagnosis between multiple myeloma or osteonecrosis: a case report
A 67-year-old female patient, diagnosed with multiple myeloma (MM), reported intense pain in the right-side mandible and hemi-face. She was undergoing third-line chemotherapy associated with the use of zoledronic acid. Clinical exams showed drainage of purulent secretion in the gingival sulcus and gingival edema in the peri-implant region. Computed tomography showed an osteolytic lesion in the right mandible, with bone sequestration adjacent to the base of a dental implant leading to MM lesion or osteonecrosis hypothesis. Jaw plasmocytoma leading to a multiple myeloma relapse diagnosis: a case report
A 46-year-old female patient was diagnosed with multiple myeloma and is undergoing treatment using the chemotherapy CTD protocol, followed by autologous stem cell transplantation and zoledronic acid use. Her complaints included paresthesia in the left mental region with increased volume. Physical examination revealed a 4.5cm rubbery nodule with intact epithelium. Cone-beam computed tomography revealed an osteolytic image of the medullary bone and cortical disruption in the lower left alveolar ridge. Osteonecrosis in the jaw associated with the use of biphosphonate after dental implant installation
Bisphosphonates are recommended to treat bone metastasis, multiple myeloma, and other diseases of bone metabolism. They inhibit the action of osteoclasts, decreasing bone resorption and turnover, and endothelial growth factor. This led to osteonecrosis, characterized by osteolytic lesions and exposure of necrotic bone, appearing spontaneously or after surgical procedures. Osteonecrosis treatment is difficult and brings morbidity to the patient. Zoledronic acid –induced severe lymphopenia
This article describes a case of severe lymphopenia following a 5 mg zoledronic acid infusion administered to treat osteoporosis. Zoledronic acid is used to treat osteoporosis, hypercalcemia, Paget ’s disease, and solid malignancies, including multiple myeloma, breast cancer, and prostate cancer. An acute phase response can be seen in 42% of patients after zoledronic acid treatment. Acute phase response may be accompanied by short-term spontaneously recovered anemia, thrombocytopenia, and se vere lymphopenia. (Source: Osteoporosis International)
