Circulating Chemerin Is Associated With Carotid Plaque Instability, Whereas Resistin Is Related to Cerebrovascular Symptomatology.
CONCLUSIONS: Low chemerin and high resistin levels were associated with carotid disease severity, suggesting that these adipokines may act as potential markers for plaque instability and stroke risk. Future studies are needed to assess causation between circulating adipokines and plaque instability. PMID: 27312219 [PubMed - as supplied by publisher]

Carotid artery stenting
Learning objectives After reading this article the reader should:Know the appropriate diagnostic workup for patients with carotid artery disease. Be able to put in perspective, based on strengths and limitations, the results of randomised trials comparing carotid artery stenting and carotid endarterectomy. Be able to identify patients that may benefit from carotid artery stenting. Introduction In western countries, stroke is the fourth leading cause of death, after heart disease, cancer and chronic respiratory diseases.1 Carotid artery disease may be responsible for 10–15% of all ischaemic strokes.2 Randomised contro...

Carotid artery stenting outcomes in dataset registries: a cause of concern or an opportunity for improvement?
Dear Editor, Roffi et al1 report that contemporary outcomes following carotid stenting (CAS) in large-scale registries were comparable to carotid endarterectomy (CEA). They also concluded that, over time, complication rates following CAS had decreased.1 We recently reviewed outcomes after >1 500 000 procedures in 21 administrative dataset registries where procedural death/stroke rates were reported for both CEA and CAS.2 One key finding was that CAS was associated with in-hospital/30-day death/stroke rates that exceeded the 3% American Heart Association (AHA) risk threshold for asymptomatic patients in 9/21 re...

Carotid stenting and endarterectomy
Stroke, either ischemic or hemorrhagic, remains the second commonest cause of death worldwide in the last decade. Etiologies for ischemic stroke (IS) vary widely. Atherothrombotic occlusion is an essential cause to which carotid artery stenosis (CAS) is a major contributor. Administration of anti-platelet agent to patients with CAS has been shown to reduce incidence of long-term IS. In additional, in patients with symptomatic CAS, clinical trials have demonstrated that carotid endarterectomy (CEA) is superior to medical therapy for prevention of future CAS-related IS.

Interventions for Extracranial Carotid Artery Stenosis: An Update
Opinion statement Clinical trials demonstrate that stroke risk after the periprocedural period is similar for carotid angioplasty and stenting (CAS) and carotid endarterectomy (CEA), making CAS an acceptable alternative to CEA. However, there tends to be a higher procedural risk of stroke for CAS than for CEA and a potentially higher rate of myocardial infarction in patients undergoing CEA as compared to CAS. Furthermore, lower rates of complications with revascularization and of post-revascularization stroke have been attributed to advances in medical management, improved surgical technique, and new devices. We ...

Impact of different perioperative intraluminal shunt insertion methods on final patient outcomes after carotid endarterectomy in a sample of 250 patients
Conclusion Surgery with modified shunt insertion timing demonstrated standard results. Due to the potential for vessel wall injury and embolization it is crucial to pay attention to shunt insertion timing in accordance with the individual course of surgery and intraoperative SEP development.

Vascular disease: Carotid artery stenosis — stenting or endarterectomy?
Nature Reviews Cardiology 13, 181 (2016). doi:10.1038/nrcardio.2016.31 Author: Irene Fernández-Ruiz Carotid artery stenting and carotid endarterectomy are used to treat patients with severe carotid stenosis to reduce the risk of stroke. Stenting, a less invasive procedure than endarterectomy, emerged as a potentially safer alternative to invasive surgery, but questions remain over the relative efficacy and

Simultaneous coronary and carotid revascularisation
Conclusion According to our experiences and results, the simultaneous performance of CEA and CABG in patients with severe coexisting carotid artery disease who require coronary revascularization has proved to be a safe and efficacious operative strategy in these high-risk patients.

Time is brain: an update.
Authors: Naylor AR Abstract The '6-month' threshold for treating symptomatic patients is obsolete. There is compelling evidence that the highest-risk period for stroke (after suffering a transient ischemic attack) is the first 2 weeks, especially the first few days, and that carotid endarterectomy (CEA) confers maximal benefit when performed early. Despite well-documented anxieties, there is increasing evidence that CEA can be performed safely within the first 7 days after onset of symptoms, although risks may be higher when performed within 48 h. The role for carotid artery stenting in the hyperacute period remain...

Early cardiovascular events in women with a history of gestational diabetes mellitus

Clopidogrel Use as Single Antiplatelet Therapy in Outpatients with Stable Coronary Artery Disease: Prevalence, Correlates and Association with Prognosis (from the CORONOR Study)
Conclusions: Our study shows that a significant proportion of stable CAD patients are treated with clopidogrel as SAPT in modern practice. Several correlates of such an attitude were identified. Our results suggest that this strategy is not beneficial as compared to aspirin alone in terms of ischaemic or bleeding events.Cardiology 2016;134:11-18

Low Stroke Rate of Carotid Stenosis Under the Guideline-Oriented Medical Treatment Compared With Surgical Treatment.
Authors: Sato K, Fujiyoshi K, Hoshi K, Noda C, Yamaoka-Tojo M, Ako J, Kumabe T Abstract Medical treatment for asymptomatic carotid artery stenosis (ACAS) has advanced recently. The outcomes of medical treatment and surgical treatment were evaluated to clarify the optimal treatment for ACAS.Patients with ACAS of ≥ 50% luminal narrowing underwent serial follow-up carotid artery ultrasonography for one year or more at the Center for Cardiovascular Disease Prevention between November 2006 and October 2013. The incidence of cardiovascular events (stroke, myocardial infarction, cardiovascular death) was examined in 64 ...
