Result of 48-Hour Approach to Symptomatic Carotid Lesions in Patients With Stable Neurologic Examinations
Timing of treatment for symptomatic carotid disease has a controversial history with current guidelines recommending carotid surgery within 48  hours up to 2 weeks. Advocating for earlier carotid endarterectomy (CEA) after stroke (cerebrovascular accident [CVA]) or while a patient is having transient ischemic attacks (TIA) can potentially provide additional benefit to acute stroke patients resulting in improved stroke and death outcomes.

Impact of Race on Perioperative Outcomes Following Carotid Endarterectomy, Transfemoral Carotid Artery Stenting, and Transcarotid Artery Revascularization
Carotid artery revascularization is performed less frequently among Black and minority populations despite a higher incidence of stroke and greater cardiovascular and stroke risk profile. Limited data exists on what impact race has on outcomes following carotid interventions. We assessed racial differences in perioperative outcomes following carotid endarterectomy (CEA), transfemoral carotid artery stenting (tfCAS), and transcarotid artery revascularization (TCAR) using the Vascular Quality Initiative database.

Do the technical features of carotid eversion affect early restenosis? Design of a prospective multicenter study
Myointimal hyperplasia restenosis remains a concern following carotid endarterectomy. It occurs in about 6% of people. The 3-year rate of stroke after restenosis is 5.2%. Several risk factors for restenosis have been identified, such as smoking, hypertension, female gender, diabetes, dyslipidemia, or small carotid diameter. The primary objective of this prospective multicenter study was to determine whether the technical characteristics of the eversion technique influence the 1-year rate of restenosis.

Reliability of tomographic 3D ultrasound in measuring internal carotid artery plaque volume
CONCLUSION: t3DUS is a reproducible imaging method and showed excellent agreement with the reference standard with respect to measuring CPV. Female and Male Patients Have Similar Outcomes After TransCarotid Artery Revascularization in Prospective Trials
Sex disparities in outcomes after carotid revascularization have long been a concern, with several studies demonstrating increased post-operative death and stroke for female patients after either carotid endarterectomy (CEA) or transfemoral stenting (TF-CAS). Adverse events after TF-CAS are higher in female patients, particularly in symptomatic cases. Our objective was to investigate outcomes after transcarotid revascularization (TCAR) stratified by patient sex hypothesizing that the results would be similar between males and females.

Complications and outcomes of carotid artery stenting in high-risk cases
In treatment of internal carotid artery stenosis, carotid artery stenting (CAS) is the standard choice for patients at risk for carotid endarterectomy (CEA). Occasionally, patients at risk of both CEA and CAS are encountered. In addition, in practice sometimes CAS is performed in patients with high risk due to patient preference and other factors. In general, symptomatic carotid artery stenosis is a risk factor for CAS1. Despite this, the American Heart Association and Stroke Association guidelines recommend CAS as an alternative to CEA for symptomatic moderate-to-severe stenosis2.

Acute ischemic stroke: The role of emergency carotid endarterectomy in isolated extracranial internal carotid artery occlusion
CONCLUSIONS: Emergency CEA in isolated eICA occlusion has proved to be a safe and effective treatment option in selected patients. CT perfusion, imaging the ischemic penumbra and quantifying the tissue suitable for reperfusion, offers a valid support in the diagnostic-therapeutic workup. Indeed, we can infer that the area of the ischemic penumbra is directly proportional to the margin of clinical improvement after revascularization, supposing that the appropriate intervention timing is respect.PMID:37594376 | DOI:10.1177/17085381231192712
