Brain Microbleeds 12 Years after Orthotopic Liver Transplantation in Val30Met Amyloidosis
Unexplained focal neurologic episodes (FNEs) can occur in patients with transthyretin-related familial amyloidotic polyneuropathy (TTR-FAP) after orthotopic liver transplantation (OLT). A patient with Val30Met FAP underwent OLT at age 34 years. Twelve years after transplantation, she presented with recurrent FNEs lasting from 10 minutes to 8 hours each, with nonuniform deficitary clinical features and variably associated with headache. Magnetic resonance imaging showed multiple brain microbleeds and diffuse contrast enhancement of the craniospinal leptomeninges consistent with amyloid deposits.

Transthyretin-type cerebral amyloid angiopathy: a serious complication in post-transplant patients with familial amyloid polyneuropathy
Liver transplantation is a well-established treatment for transthyretin (TTR)-type familial amyloid polyneuropathy (TTR-FAP).1 According to data in the Familial Amyloidotic Polyneuropathy World Transplant Registry (http://www.fapwtr.org/ram_fap.htm), more than 2000 liver transplantations have been performed to date in 19 countries. Transplantation replaces the variant TTR gene with the wild-type gene in the liver, the main source of serum circulating TTR. The serum concentration of variant TTR decreases rapidly, reaching almost zero after the operation. The effects of liver transplantation on neuropathy are evident as its ...

ADAMTS13 and von Willebrand Factor in Thrombotic Thrombocytopenic Purpura.
Abstract Pathogenesis of thrombotic thrombocytopenic purpura (TTP) was a mystery for over half a century until the discovery of ADAMTS13. ADAMTS13 is primarily synthesized in the liver, and its main function is to cleave von Willebrand factor (VWF) anchored on the endothelial surface, in circulation, and at the sites of vascular injury. Deficiency of plasma ADAMTS13 activity (<10%) resulting from mutations of the ADAMTS13 gene or autoantibodies against ADAMTS13 causes hereditary or acquired (idiopathic) TTP. ADAMTS13 activity is usually normal or modestly reduced (>20%) in other forms of thrombotic microangi...

Preexisting Atrial Fibrillation and Cardiac Complications After Liver Transplantation
Conclusion: Although patients with AF have a higher incidence of intraoperative cardiac events, a higher cardiovascular morbidity and a complicated postoperative course, this did not affect overall graft and patient survival. This article is protected by copyright. All rights reserved.

Ten-point plan to tackle liver disease published
"Doctors call for tougher laws on alcohol abuse to tackle liver disease crisis," The Guardian reports. But this is just one of 10 recommendations for tackling the burden of liver disease published in a special report in The Lancet. The report paints a grim picture of an emerging crisis in liver disease in the UK, saying it is one of the few countries in Europe where liver disease and deaths have actually increased rapidly over the last 30 years. It concludes with 10 recommendations to tackle the burden of liver disease. The media has approached the recommendations from many different angles, with many sources only ...

Central Nervous System Complications After Liver Transplantation: Common but Mostly Transient Phenomena
Conclusions: CNSc are common and relevant complications after OLT. Patients after OLT, especially with risk factors, should undergo regular standardized neurological examination that would allow early detection of these complications. Liver Transpl , 2014. © 2014 AASLD.

High early cardiovascular mortality after liver transplantation
In conclusion, we provide the first multicenter prognostic model for the prediction of early post‐LT CVD death, the most common cause of early post‐LT mortality in the current transplant era. However, evaluations of additional CVD‐related variables not collected by the OPTN are needed in order to improve the model's accuracy and potential clinical utility. Postoperative Stroke Following Administration of Intraoperative Recombinant Tissue Plasminogen Activator for the Treatment of Intracardiac Thrombus During Liver Transplantation: A Report of Two Cases
MASSIVE INTRACARDIAC AND PULMONARY thromboembolism have an extremely high mortality during liver transplantation.1,2 The authors describe 2 cases in which intraoperative use of tissue plasminogen activator was able to reduce the thrombus size and improve hemodynamics during liver transplantation. Although both patients survived the surgery, they suffered neurologic sequelae postoperatively. The authors discuss the current understanding of the pathophysiology, diagnosis, and management of intracardiac thromboembolism during liver transplantation.

Effects of edaravone, a radical scavenger, on hepatocyte transplantation
ConclusionsEdaravone administration during HTx can suppress apoptosis near the transplanted cells, increasing engraftment. These studies indicate its potential usefulness for future clinical application.

Using Drug-Intoxicated Deaths as Potential Organ Donors: Impression of Attendees at the American College of Medical Toxicology 2014 Annual Scientific Meeting.
CONCLUSIONS: Medical toxicologists have some reservation in recommending solid organs for transplantation from deaths from cocaine and carbon monoxide. Given the decrease in potential organ donors from typical methods of death, further work is needed to promote organ donation in deaths related to acute poisoning. High early cardiovascular mortality following liver transplantation
Abstract Cardiovascular disease (CVD) contributes to excess long‐term mortality after liver transplantation (LT), however little is known about early post‐operative CVD mortality in the current era. In addition, there is no model to predict early post‐operative CVD mortality across centers. We analyzed adult recipients of primary LT in the Organ Procurement and Transplantation Network (OPTN) database between February 2002 and December 2012 to assess prevalence and predictors of early (30‐day) CVD mortality, defined as death from arrhythmia, heart failure, myocardial infarction, cardiac arrest, thromboembolism, and/...

Transplants give Garden Plain man a second chance at life
Chris Simon’s bucket list wasn’t anything fancy. He simply wanted to be able to ride a bike again. Or take a swim. Or go camping with friends. “Stuff I used to do all the time,” Simon said. “And all of a sudden it was taken away.” Those once simple joys were stolen by cystic fibrosis, the same disease that claimed his younger sister Emily’s life in 2009. The Simon family on Sunday will join millions of others around the world in celebrating Easter, a day symbolizing new beginnings. The holiday will have special significance for Chris, 26, who has another chapter dawning in his life after a double-lung/liver t...

Quality of Life in Liver Transplant Patients with Neurologic Complications. (P4.031)
Conclusions: Our analysis shows that neurological complications worsen the quality of life of liver transplant patients. Especially regarding perception of health, ability to function independently due to physical and mental health limitations. Pain is a limiting condition in this group. Transplant patients who suffer neurological complications should be carefully followed up to reduce the impact on quality of life, in order to promote the ultimate goal of transplant.Disclosure: Dr. Gonzalez Toledo has nothing to disclose.
