Letter to Editor on “Lacunar Stroke in Cryptococcal Meningitis: Clinical and Radiographic Features”
We read with much pleasure the article “Lacunar Stroke in Cryptococcal Meningitis: Clinical and Radiographic Features” by Vela et al published in the Journal of Stroke and Cerebrovascular Disease.1 Authors have done a commendable job at describing the clinical and radiographical details of stroke in a cohort of patients with cryptoco ccal meningitis. Authors have collected extensive data over 18years from the University of Colorado Hospital, and have identified 42 patients. Among these, 32 (76%) of the patients had available neuroimaging.

Cryptococcal meningitis carries a high mortality, and survivors are left with considerable neurologic sequelae and marked disability. Outcomes after coverage of lenticulostriate vessels by flow diverters: a multicenter experience.
CONCLUSIONSThe use and versatility of flow diversion is increasing, and safety data are continuing to accumulate. Here, the authors provide early data on the safety of covering lenticulostriate vessels with flow diverters. The authors concluded that the coverage of these perforators does not routinely lead to clinically significant ischemia when dual antiplatelet therapy is continued for 6 months. Contralateral acute vascular occlusion following revascularization surgery for moyamoya disease.
CONCLUSIONSAcute occlusion of the ICA on the contralateral side from an STA-MCA bypass is a rare, but potentially serious, complication of revascularization surgery for MMD. It highlights the importance of the hemodynamic interrelationships that exist between the two hemispheres, a concept that has been previously underappreciated. Induced hypertension during the acute period may provide adequate cerebral blood flow via developing collateral vessels, and good outcomes may be achieved with aggressive supportive management and expedited contralateral revascularization.
