Fibromuscular dysplasia incidentally identified with computed tomography angiography
A 78-year-old woman had presented to the emergency room with altered mental status after the inadvertent consumption of a cannabis product. A stroke workup was initiated, including computed tomography angiography, which demonstrated severe tortuosity and a beaded appearance of her bilateral internal carotid arteries (A/Cover and B). The radiographic imaging findings, including alternating areas of bulging and narrowing, were consistent with fibromuscular dysplasia (FMD; C). The patient denied any history of transient ischemic attack, cerebrovascular accident, amaurosis fugax, or chronic kidney issues.

Acute and subacute neurovascular impact of cryptogenic air emboli
CONCLUSION: This case highlights that cerebral air emboli can cause delayed ischemia that may not be appreciated on initial imaging. Discussion Vascular DR lesions of the splenium were more common than CLOCCs and other etiologies in this cohort. While splenial DR lesions can present a clinical challenge, their associated clinical and radiographic characteristics may predict outcome and guide prognosis.

ConclusionThe implementation of AI and ML algorithms to supplement clinical practice has conferred a high degree of accuracy, efficiency, and expedited detection in the clinical and radiographic evaluation and management of ischemic and hemorrhagic strokes, AVMs, and aneurysms. Initial experience with minimally invasive endoscopic evacuation of intracerebral hemorrhage in the setting of radiographic herniation
Spontaneous intracerebral hemorrhage (ICH) can rapidly result in cerebral herniation, leading to poor neurologic outcomes or mortality. To date, neither decompressive hemicraniectomy (DH) nor hematoma evacuation have been conclusively shown to improve outcomes for comatose ICH patients presenting with cerebral herniation, with these patients largely excluded from clinical trials. Here we present the outcomes of a series of patients presenting with ICH and radiographic herniation who underwent emergent minimally invasive (MIS) ICH evacuation.
