Significance of Hematoma Shape and Density in Intracerebral Hemorrhage: The Intensive Blood Pressure Reduction in Acute Intracerebral Hemorrhage Trial Study Clinical Sciences
Conclusions— Irregular shape, but not heterogeneous density, is independently associated with poor outcome after ICH. Clinical Trial Registration— URL: http://www.clinicaltrials.gov. Unique identifier: NCT00716079.

Initial Conservative Management of Severe Hemispheric Stroke Reduces Decompressive Craniectomy Rates
Conclusions Delayed DC for hemispheric stroke patients managed under protocol in the neurocritical care unit is a safe alternative to early, prophylactic DC for adults with severe hemispheric stroke. This strategy reduced DC rates by 60 % without an excess of death or survival with severe disabilities.

What Genetics are Associated with Multiple Sclerosis?
Discussion Multiple sclerosis (MS) is “a chronic degenerative, often episodic disease of the central nervous system marked by patchy destruction of the myelin that surrounds and insulates nerve fibers, usually appearing in young adulthood and manifested by one or more mild to severe neural and muscular impairments, as spastic weakness in one or more limbs, local sensory losses, bladder dysfunction, or visual disturbances.” It is a chronic disease and therefore symptoms must occur more than once. The first episode is called an acute demyelinating attack. Fifteen to forty-five percent of children with their first...

How one ED uses telemedicine in the ambulance
When you think of telemedicine, what comes to mind? Often the answer is a split screen—physician and patient in separate locations on their computers or tablets. But one health system has shown the true breadth of telemedicine’s reach by using the technology to treat patients during the critical early moments of a stroke. Find out how. The risk of damage and disability in patients who are experiencing a stroke increases with any delay in care delivery. Two emergency physicians at the University of Virginia (UVA) Health System understood the need for speed when it comes to caring for patients in the midst of acute str...

A comprehensive analysis of metabolic changes in the salvaged penumbra
Conclusion Decreased glutamate, creatine, and NAA concentrations are associated with poor neurological outcome at 24 h and greater disability at 3 months. The significant metabolic variation in salvaged tissue may potentially explain some of the variability seen in stroke recovery despite apparently successful reperfusion.

Pre-hospital notification is associated with improved stroke thrombolysis timing.
Conclusion Pre-hospital notification is associated with earlier stroke physician review, CT imaging and delivery of thrombolysis. Referral to an out of hours thrombolysis service was not associated with additional delay. PMID: 26517096 [PubMed - in process]

Evidence of increased brain amyloid in severe TBI survivors at 1, 12, and 24 months after injury: report of 2 cases.
Abstract Traumatic brain injury (TBI) is a major risk factor for Alzheimer's disease. With respect to amyloid deposition, there are no published serial data regarding the deposition rate of amyloid throughout the brain after TBI. The authors conducted serial (18)F-AV-45 (florbetapir F18) positron emission tomography (PET) imaging in 2 patients with severe TBI at 1, 12, and 24 months after injury. A total of 12 brain regions were surveyed for changes in amyloid levels. Case 1 involved a 50-year-old man who experienced a severe TBI. Compared with the 1-month time point, of the 12 brain regions that were surveyed, a ...

Can You Think Yourself Into A Different Person?
For years she had tried to be the perfect wife and mother but now, divorced, with two sons, having gone through another break-up and in despair about her future, she felt as if she’d failed at it all, and she was tired of it. On 6 June 2007 Debbie Hampton, of Greensboro, North Carolina, took an overdose of more than 90 pills – a combination of ten different prescription drugs, some of which she’d stolen from a neighbor’s bedside cabinet. That afternoon, she’d written a note on her computer: “I’ve screwed up this life so bad that there is no place here for me and nothing I can contr...

Malignant MCA Infarction: Pathophysiology and Imaging for Early Diagnosis and Management Decisions
Background: Malignant middle cerebral artery infarction is a devastating condition, with up to 80% mortality in conservatively treated patients. The pathophysiology of this stroke is characterized by a large core of severe ischemia and only a relatively small rim of penumbra. Due to the fast development of irreversible morphological damage, cytotoxic edema occurs immediately in a large portion of the ischemic territory. The subsequent damage of the tight junctions leads to the breakdown of the blood brain barrier and vasogenic brain edema, resulting in space-occupying brain swelling. The progressive vasogenic edema reaches...

When Stroke Happens... at Age 27
Ever wonder what it's like to experience a stroke? Avid runner Emily Welbourn blogs about the day she had a stroke while running a race. At the sound of the starting gun, I charge forward with the other runners selected from around the world. In spite of being at peak physical health, I slowly realize my pace isn't sustainable. The one-mile marker is now ahead, I've got this. Just keep moving. Suddenly I am stabbed above the eyebrow...but no one is within arm's reach. Blindsided, I squeeze my eyes shut for a moment to tamper the pain, and the invisible knife is dragged across the top of my head down to my neck. Never i...
