Acute Hearing Loss Caused by Decreasing Anterior Inferior Cerebellar Arterial Perfusion in a Patient with Vertebral Artery Stenosis
We report a case of bilateral hearing loss caused by decreased vascular flow in the anterior inferior cerebellar artery (AICA) territory. A 74-year-old man who experienced right hearing loss 5 months ago presented with bilateral deafness and right cerebellar ataxia; however, no ischemic lesion was detected in the bilateral AICA area. After stroke treatment, hearing loss was improved. One month later, we obtained blood flow improvement in the left AICA territory on single –photon-emission computed tomography and vertebral artery stenosis on magnetic resonance angiography.

The Posterior Fossa and Foreign Accent Syndrome: Report of Two New Cases and Review of the Literature.
Abstract Foreign accent syndrome is a rare motor speech disorder that causes patients to speak their language with a non-native accent. In the neurogenic condition, the disorder develops after lesions in the language dominant hemisphere, often affecting Broca's area, the insula, the supplementary motor area and the primary motor cortex. Here, we present two new cases of FAS after posterior fossa lesions. The first case is a 44-year-old, right-handed, Dutch-speaking man who suffered motor speech disturbances and a left hemiplegia after a pontine infarction. Quantified SPECT showed a bilateral hypoperfusion in the i...

Abnormal neurovascular coupling during status epilepticus migrainosus in Sturge-Weber syndrome
Sturge-Weber syndrome is a rare, sporadic, neurocutaneous disorder classically characterized by a facial nevus in the trigeminal distribution (port wine stain), leptomeningeal angiomatosis, and glaucoma, although intracranial changes can be present without cutaneous involvement.1 Seizures occur in up to 75% of individuals, are often associated with headache, and stroke-like episodes causing prolonged neurologic deficits are also described.1,2

Case report: Rapid improvement of crossed cerebellar diaschisis after intravascular laser irradiation of blood in a case of stroke
Rationale: Crossed cerebellar diaschisis (CCD) is a poor prognostic factor after stroke because without immediate cerebral reperfusion no further improvements in the patient's condition can be achieved. We investigated the clinical effects of intravascular laser irradiation therapy (ILIB) on CCD and evaluated the therapeutic effect in the sub-acute post-stroke stage. Patient concerns: The 77-year-old male with cerebral infarction in the territory of the right anterior cerebral artery only underwent conservative treatment including hydration and aspirin in the acute post-stroke stage. Diagnosis: He was diagnosed as stroke...

The masaryk hospital extracranial –intracranial bypass study
AbstractPatients with symptomatic internal carotid artery (ICA) occlusion with haemodynamic impairment are at higher risk of ischaemic stroke, and they require treatment. There are two main options: the best medical treatment and an extracranial –intracranial bypass. The aims of this study are to analyse the 30-day and 2-year risk of stroke and death in patients with extracranial–intracranial bypass performed by our department and to compare our results with major published studies. This retrospective study enrolled patients who underwe nt surgery from 1998 to 2012. Inclusion criteria were the following: (1) radiologic...

Assessment of Hemodynamic Compromise Using Computed Tomography Perfusion in Combination with 123I-IMP Single-Photon Emission Computed Tomography without Acetazolamide Challenge Test
In this study, we investigated whether CT perfusion in combination with resting state 123I-IMP SPECT could be used instead of the acetazolamide challenge test to evaluate hemodynamic compromise in patients with atherosclerotic occlusive disease.

Erythropoietin Pretreatment of Transplanted Endothelial Colony-Forming Cells Enhances Recovery in a Cerebral Ischemia Model by Increasing Their Homing Ability: A SPECT/CT Study
Conclusion: Priming with erythropoietin before cell transplantation is an efficient strategy to amplify the migratory and engraftment capacities of ECFCs and their beneficial impact on BBB disruption, apoptosis, and CBF.

Clinical implications for Translocator Protein (TSPO) imaging using 123ICLINDE SPECT: A feasible approach
Conclusions The preliminary results imply that [123I]CLINDE-SPECT is a feasible approach for TSPO imaging in neurological disorders associated with microglial activation and/or increased TSPO expression. The development of methods that do not need arterial blood sampling for quantification of TSPO will be important to ease implementation in a daily clinical setting. Further clinical studies are needed to determine the precise role of TSPO imaging in predicting clinical outcome and treatment response.

Early detection of doxorubicin-induced cardiotoxicity using the matrix metalloproteinase (MMP) targeted radiotracer, 99mTc-RP805.
Conclusions Increases in myocardial MMP activity precede clinically used echocardiographic indices for early detection of cardiotoxicity, including ejection fraction and global myocardial strain. Therefore, 99mTc-RP805 may serve as a promising approach for early detection of DOX-induced cardiotoxicity when combined with SPECT/CT imaging.

Correlation of Transcranial Doppler Breath Holding Test and Acetazolamide SPECT Scan for Anterior Circulation Steno-Occlusive Disease (P6.019)
Conclusions: There was a modest correlation between TCD-R and acetazolamide SPECT scan for anterior circulation steno-occlusive disease in our patient cohort. TCD-R demonstrated higher sensitivity for the detection of poor cerebrovascular reserve than acetazolamide SPECT. Further studies are needed to determine which study is more useful in the management of this high-risk population. Disclosure: Dr. Parikh has nothing to disclose. Dr. Cerejo has nothing to disclose. A Case of Transient Global Amnesia: A Review and How It May Shed Further Insight into the Neurobiology of Delusions
Conclusion In closing, our patient’s episode of TGA combined with her emotional and perceptual response lends credence to the proposal of a “fear/paranoia” circuit in the genesis of paranoid delusions—a circuit incorporating amygdala, frontal, and parietal cortices. Here, neutral or irrelevant stimuli, thoughts, and percepts come to engender fear and anxiety, while dysfunction in frontoparietal circuitry engenders inappropriate social predictions and maladaptive inferences about the intentions of others.[54] Hippocampus relays information about contextual information based on past experiences and the current situat...
