Undiagnosed Heterotopic Pregnancy, Maternal Hemorrhagic Shock, and Ischemic Stroke in the Intrauterine Fetus
Abstract Heterotopic pregnancy occurs rarely following natural conception; however, intrauterine embryo transfer following IVF is a known risk factor for its occurrence. A 29‐year‐old woman presented with acute abdomen at 14w5d gestation following IVF‐ET treatment. A ruptured heterotopic gestation in the left fallopian tube was identified at laparoscopy and treated by salpingectomy. Subsequently, at 21‐week gestation, routine sonogram demonstrated bilateral ventriculomegaly in the intrauterine fetus. Intraabdominal pressure, cardiac index and vascular resistance during hyperthermic intraperitoneal chemotherapy - a prospective observational study.
CONCLUSIONS: Increased intraabdominal pressure during HIPEC was comparable to pneumoperitoneum. Haemodynamic changes however were opposed with a decrease in SVRI and a compensative increase in CI. Current guidelines for anaesthetic management in patients undergoing HIPEC are mainly based on findings from laparoscopic surgery and should therefore be reconsidered critically. Roux-en-Y gastric bypass alleviates hypertension and is associated with an increase in mid-regional pro-atrial natriuretic peptide in morbid obese patients
Conclusions: LRYGB resulted in a significant 24BP reduction and a substantial increase in MRproANP plasma concentrations in hypertensive, obese patients 6 weeks after surgery, suggesting a causal link between obesity–hypertension and altered release/degradation of cardiac natriuretic peptides.

In some studies, more than 30% of patients who underwent either vaginal surgery or abdominal surgery reported inadequate pain control at postoperative days 3 and 7. Intraoperative circulatory management using the FloTracTM system in laparoscopic liver resection
ConclusionThe average SVV value during laparoscopic liver transection (mean, 17.0%) exceeded the conventional cut‐off value, but in this study, no perioperative complications developed, which enabled safe management. We might be able to manage appropriate fluid control using FloTrac system in patients with laparoscopic liver resection. Therefore, it is necessary to set the target SVV and conduct prospective trials to verify the safety margin for intraoperative management in the future.

Factors Associated with Conversion from Laparoscopic to Open Colectomy using the National Surgical Quality Improvement Program (NSQIP) Database
ConclusionsMultiple significant factors for conversion from laparoscopic to open colectomy were identified. A novel finding was the increased risk of conversion for underweight patients. As laparoscopic colectomy is become increasingly utilized, factors predictive of conversion to open procedures should be sought via large, national cohorts.This article is protected by copyright. Gastroesophageal Reflux in Chronic Cough and Cough Syncope and the Effect of Antireflux Treatment: Case Report and Literature Review.
CONCLUSION: For chronic cough and cough syncope of unknown cause, the GER assessment could be valuable. In treating well-selected GER-related chronic cough and cough syncope, PPI, SRF, and LF can be considered. Effects of pneumoperitoneum induced at various pressures on cardiorespiratory function and working space during laparoscopy in cats.
Conclusions and Clinical Relevance-Values of cardiopulmonary variables were largely unchanged by induction of pneumoperitoneum in healthy cats up to an IAP of 8 mm Hg, and no clinically important increases in working space were evident at an IAP of 15 versus 8 mm Hg. These findings provide little justification for use of IAPs > 8 mm Hg in healthy cats undergoing laparoscopic procedures; however, whether the situation is similar in diseased or elderly cats remains to be determined.
