Laparoscopically-confirmed endometriosis and risk of incident stroke: a prospective cohort study
Prior research has suggested that women with endometriosis are at greater risk of cardiovascular disease later in life, including myocardial infarction and hypertension. However, there has been limited research on endometriosis and risk of stroke, which may have similar underlying pathophysiologic mechanisms. Therefore, our objective was to prospectively investigate the association between laparoscopically-confirmed endometriosis and risk of incident stroke during 28 years of follow-up.

Perioperative hemodynamic optimization in laparoscopic sleeve gastrectomy using stroke volume variation to reduce postoperative nausea and vomiting
CONCLUSION: No significant benefit is found in reducing PONV by using GDFT in patients undergoing LSG, although GDFT effectively avoids excessive volume of fluid administration. PONV incidence appears to be higher with intraoperative colloid infusion for GDFT during LSG. Further investigation is warranted to elucidate the mechanism underlying PONV in postoperative LSG.PMID:34247980 | DOI:10.1016/j.soard.2021.06.004

Longitudinal Gastrectomy for Nonbariatric Indications
CONCLUSION: LLG seems to be an appropriate intervention for various gastric pathologies. Training of residents and fellows in the minimally invasive surgical steps of LLG is encouraged.PMID:34035960 | PMC:PMC8124009 | DOI:10.1155/2021/9962130

Routine preoperative resting echocardiography does not predict adverse cardiopulmonary events after bariatric surgery
CONCLUSION: A routine resting preoperative echocardiography added little to the cardiopulmonary risk stratification of patients who underwent bariatric surgery. High-risk patients were identifiable based on their medical history, particularly those with a known history of coronary artery disease and coronary revascularization.PMID:33637417 | DOI:10.1016/j.soard.2021.01.021

Cerebral infarction After Laparoscopic Right Lung Wedge or Segment Resection: A Report of Four Cases
Several cases have been reported of patients who experienced cerebral infarction following thoracoscope left lobectomy. Compared with right lung surgery, the pulmonary veins stump after left lobe surgery were longer and thrombosis was more likely. Besides, cases of cerebral infarction after right lung surgery are rarely reported. Left lobectomy is therefore considered as the main risk factor for postoperative cerebral infarction. However, here we report four cases of cerebral infarction after thoracoscopic wedge or segment resection of right lobe, which cause less damage to the pulmonary vein compared with lobectomy.

Minimizing postoperative pulmonary complications in thoracic surgery patients
Purpose of review Quantification and optimization of perioperative risk factors focusing on anesthesia-related strategies to reduce postoperative pulmonary complications (PPCs) after lung and esophageal surgery. Recent findings There is an increasing amount of multimorbid patients undergoing thoracic surgery due to the demographic development and medical progress in perioperative medicine. Nevertheless, the rate of PPCs after thoracic surgery is still up to 30–50% with a significant influence on patients’ outcome. PPCs are ranked first among the leading causes of early mortality after thoracic surgery. Altho...

Laparoscopic instrument for female surgeons: an innovative model for endoscopic purse-string suture.
Authors: Kono E, Taniguchi K, Lee SW, Ohdaira T, Uchiyama K Abstract Commercially available laparoscopic instruments are not designed for female surgeons. We redesigned the endoscopic flexible head purse-string suture instrument for improved use by female surgeons. The weight, total length, and diameter of the swing head handle and clump handle were reduced (786 to 565 g, 715 to 700 mm, 70 to 50 mm, and 30 to 25 mm, respectively). Stroke of the slide for firing and release of the lever was reduced from 92 to 83 mm. This is the first step toward an ergonomic surgical device that considers physical differen...
