A nationwide population-based retrospective cohort study: increased risk of acute coronary syndrome in patients with ankylosing spondylitis.
Conclusions: AS patients are at higher risk of ACS compared with non-AS subjects. Management of CV risk factors should be taken into account for the treatment of patients with AS, especially for patients with co-morbidities of hypertension, DM, and cancer. Prevalence of comorbidities in rheumatoid arthritis and evaluation of their monitoring: results of an international, cross-sectional study (COMORA)
Conclusions Among RA patients, there is a high prevalence of comorbidities and their risk factors. In this multinational sample, variability among countries was wide, not only in prevalence but also in compliance with recommendations for preventing and managing these comorbidities. Systematic measurement of vital signs and laboratory testing detects otherwise unrecognised comorbid conditions.

Changes in mortality patterns following total hip or knee replacement over the past two decades: A nationwide cohort study
Conclusion: Mortality rates following elective THR and TKR have decreased substantially since the early 90s, despite operated patients having more comorbidity. These findings are reassuring for patients undergoing elective THR or TKR. © 2013 American College of Rheumatology.

Changes in Mortality Patterns Following Total Hip or Knee Arthroplasty Over the Past Two Decades: A Nationwide Cohort Study
ConclusionMortality rates following elective THA and TKA have decreased substantially since the early 1990s, despite patients having more presurgical comorbidity. These findings are reassuring for patients undergoing elective THA or TKA.

Mortality and Cardiovascular Burden of Systemic Lupus Erythematosus in a US Population-based Cohort.
CONCLUSION: This incident SLE cohort demonstrated nearly doubled mortality and CVD event hazards compared to age-matched and sex-matched comparisons, even after accounting for higher CVD events in the 2 years preceding SLE diagnosis. This raises research questions regarding delayed SLE diagnosis versus accelerated CVD prior to SLE, particularly in older-onset SLE. PMID: 24532834 [PubMed - as supplied by publisher]

Tumor Necrosis Factor α Inhibitor Use and Decreased Risk for Incident Coronary Events in Rheumatoid Arthritis
ConclusionUse of TNFα inhibitors is associated with a decreased risk for CAD in RA; the risk decreases further with long‐term use. This should be considered when weighing the risks versus benefits of these medications.

Factors Associated with Major Cardiovascular Events in Patients with Systemic Necrotizing Vasculitides: Results of a Longterm Followup Study.
CONCLUSION: These results suggest that factors associated with a higher MCVE risk in patients with SNV are NCEP/ATP III-defined high-risk status and BMI > 30 kg/m(2). Carotid IMT could help identify patients with SNV at risk of early MCVE. PMID: 24584925 [PubMed - as supplied by publisher]

A16: Long‐term follow‐up of 36 Pediatric Antiphospholipid Syndrome Patients: A Retrospective Study
Conclusion:Despite of prophylaxis with anticoagulants, new thrombotic events occurred in 25% of the patients and reaching the ideal prothrombin time to prevent thrombotic episodes was the major challenge, as nearly half of the patients had hemorrhagic events. Studies are necessary to identify ideal parameters for treating and monitoring children with antiphospholipid syndrome.

The association between inflammatory markers, serum lipids and the risk of cardiovascular events in patients with rheumatoid arthritis
Conclusions This study provides evidence supporting the hypothesis that RA-related systemic inflammation plays a role in determining cardiovascular risk and a complex relationship between LDL and cardiovascular risk.

Cerebral large vessel vasculitis in systemic lupus erythematosus.
We report the case of a female patient with the diagnosis of SLE. She presented with stroke-like symptoms, headache and vertigo, and palpable purpura on her legs. Further investigations revealed that she suffered from both vasculitis of the cerebral large vessels and coexisting cutaneous small-vessel vasculitis. Response to: 'Overweight/obesity and vitamin D deficiency contribute to the global burden of low back pain' by Professor Grant
We would like to thank William B Grant for his letter1 on our paper ‘The global burden of low back pain: estimates from the Global Burden of Disease 2010 study’.2 We fully agree with Professor Grant on the enormous potential that population-wide interventions have for reducing the global burden of low back pain. While more research is needed, the evidence base for several population-based interventions is already well established. For example, globally, and in low-income and middle-income countries in particular, population-based strategies that increase physical activity and reduce obesity are likely to have a...

Non-steroidal anti-inflammatory drugs and risk of cardiovascular disease in patients with rheumatoid arthritis: a nationwide cohort study
Conclusions The cardiovascular risk associated with NSAID use in RA patients was modest and significantly lower than in non-RA individuals. Moreover, only a few of the individual NSAIDs were associated with increased cardiovascular risk. NSAID use should be assessed in the individual patient based on the indication for pain relief and risk factors for adverse effects, and not automatically be avoided due to concerns of severe cardiovascular outcomes alone.

The effect of body mass index on the risk of post-operative complications during the 6 months following total hip replacement or total knee replacement surgery
Summary: Objective: To assess the effect of obesity on 6-month post-operative complications following total knee (TKR) or hip (THR) replacement.Design: Data for patients undergoing first THR or TKR between 1995 and 2011 was taken from the Clinical Practice Research Datalink. Logistic regression was used to assess whether body mass index (BMI) was associated with 6-month post-operative complications [deep vein thrombosis or pulmonary embolism (DVT/PE), myocardial infarction (MI), stroke, respiratory infection, anaemia, wound infection, urinary tract infection or death] after controlling for the effects of age, gender, smoki...

Adverse outcomes after major surgery in patients with systemic lupus erythematosus: a nationwide population-based study
Conclusions SLE significantly increased the risks of surgical patients for overall major complications and mortality after major surgery. Our findings demonstrated the need for integrated care and revised protocols for perioperative management to improve outcomes for surgical patients with SLE.
