Comparison of magnetic resonance spectroscopy (MRS) with arterial spin labeling (ASL) in the differentiation between mitochondrial encephalomyopathy, lactic Acidosis, plus stroke-like episodes (MELAS) and acute ischemic stroke (AIS)
Publication date: September 2018Source: Journal of Clinical Neuroscience, Volume 55Author(s): Xiaodi Li, Yuzhou Wang, Zhanhang Wang, Jianjun Lu, Yan Xu, Jinlong Ye, Zuying Kuang, Bo Li, Mengqiu Pan, Wenming Chen, Shuisheng Lu, Ziyang Zhou, Lina Cheng, Honghao WangAbstractTo compare the utility and limitation of magnetic resonance spectroscopy (MRS) and arterial spin labeling (ASL) in the differentiation between mitochondrial encephalomyopathy, lactic acidosis, plus stroke-like episodes (MELAS) and acute ischemic stroke (AIS), a retrospective review of 17 MELAS and 26 AIS patients were performed. In all patients both MRS an...

CT perfusion and EEG patterns in patients with acute isolated aphasia in seizure-related stroke mimics
Stroke mimic (SM) is a set of conditions with clinical presentation similar to that of an acute ischemic stroke (AIS), albeit not caused by an ischemic event [1]. From 1% to 41% of patients presenting stroke-like symptoms at admission to the Emergency Department are actually SM [2]. The most common SM etiologies are seizure (38%), migraine with aura (37%), and conversion disorder (21%) [3]. Other conditions of SM are metabolic, infectious, neurodegenerative disorder, peripheral neuropathy and syncope [3].

Younger age at stroke onset but not thrombolytic treatment predicts poststroke epilepsy: An updated meta-analysis
ConclusionDespite limitations due to the uneven quality and design of the studies, the present meta-analysis confirms that cortical involvement, hemorrhagic component, and ES are associated with a higher risk of PSE. In this update, younger age at stroke onset but not thrombolytic treatment seems to increase the risk for PSE.This article is part of the Special Issue “Seizures & Stroke”.

Burden of Epileptiform Activity Predicts Discharge Neurologic Outcomes in Severe Acute Ischemic Stroke
ConclusionsElectrographic seizures and periodic and rhythmic patterns in patients with AIS are associated with worse outcomes in a dose-dependent manner. Future studies are needed to assess whether treatment of this EEG activity can improve outcomes.

Stroke, Epilepsy, and Antiepileptic Drugs as the Etiology of Major Depressive Disorder: A Case Report and Review.
Authors: Almazan AN, Weisholtz DS, Almeida M Abstract Lateralized periodic discharges (LPDs) are a rare type of epileptiform activity. The authors of this brief report describe the association of LPDs with acute catastrophic depression in a previously healthy patient who had a series of neurobiological insults caused by a stroke and its treatment that led to the development of significant psychopathology. Through this case of new and debilitating depression that followed a recent stroke, subsequent development of LPDs, and treatment with levetiracetam, the authors highlight the complex interplay of structural, elec...

Cardiogenic cerebral infarction in the parietal lobe predicts the development of post-stroke epilepsy
Seizures are a common neurological disorder in older people with the cumulative incidence of epilepsy through age 74 years was 3.0% [1]. Particularly traumatic brain injuries and abnormal cerebral lesions are associated with an increased risk for developing epileptic seizures. Currently, such symptomatic etiologies account for 30% –49% of all unprovoked seizures and epilepsy [1,2]. Additionally, stroke is a predominant epileptogenic condition and the main cause of seizures in the elderly. Seizures after stroke can be divided into two broad categories: early-onset seizures occurring within 2 weeks and late-onset seizures ...

Effects of atorvastatin and aspirin on post-stroke epilepsy and usage of levetiracetam
Conclusion: Atorvastatin and aspirin co-treatment with levetiracetam can reduce epilepsy in PSE patients and reduce the dosage of levetiracetam required for effective control of PSE.

Endovascular Treatment of Acute Ischemic Stroke in Clinical Practice: Analysis of Workflow and Outcome in a Tertiary Care Center
Conclusion: The analysis revealed several starting points for an improvement in the workflow of thrombectomy in our center. Analyses of workflow and treatment results should be carried out regularly to identify the potential for optimization of operational procedures and selection criteria for patients who could benefit from endovascular treatment.

Meningioma preceding CASPR2 antibody limbic encephalitis with a stroke mimic: A case report
We report a contactin-associated protein-like 2-antibody (CASPR2-Ab)-positive patient who presented with atypical LE. Diagnoses: CASPR2-Ab-positive LE was the presumed diagnosis. Re-evaluation of cerebrospinal fluid (CSF) samples revealed autoantibodies targeting CASPR2 at an immunoglobulin G titer of 1:1. The clinical presentation of subacute onset seizures, abnormal electroencephalography, hypermetabolism on positron emission tomography, good immunotherapy response, and the presence of specific antibodies in serum supports a diagnosis of autoimmune LE. Intervention: The patient received glucocorticoids (1...

Cerebral Perfusion in Pediatric Stroke: Children Are Not Little Adults
Cerebral perfusion imaging provides useful information about the hemodynamic state of the brain that is relevant to a number of neurologic conditions, including stroke, epilepsy, and brain tumors. Multiple imaging modalities have been developed to evaluate brain perfusion, primarily by the use of different tracers to measure cerebral blood volume and cerebral blood flow. Here, we provide an overview of magnetic resonance imaging perfusion techniques; summarize the role of perfusion imaging in adult stroke; describe changes in cerebral blood flow over childhood; and discuss the relevance and future directions of perfusion i...

Asymmetric distribution of enlarged perivascular spaces in centrum semiovale may be associated with epilepsy after acute ischemic stroke
CONCLUSIONS: Asymmetric distribution of EPVS in CS may be an independent risk factor and a novel imaging biomarker for the development of PSE. Further studies to understand the mechanisms of this association and confirmation with larger patient populations are warranted.PMID:34981639 | DOI:10.1111/cns.13786

Analysis of psychiatric consultations in the department of neurology and stroke unit: diagnosis and therapy
CONCLUSIONS: Patients in the department of neurology and stroke unit underwent a psychiatric consultation most frequently. Optimizing the care of aneurological patient is related to close cooperation of a neurologist and a psychiatrist, as well as developing and updating common positions for the management of selected disease entities.PMID:34997738 | DOI:10.12740/PP/OnlineFirst/116320
