The Impact of a Stroke Prevention Clinic after Discharge for TIA/ Stroke: University Hospitals Case Medical Center Comprehensive Stroke Center Experience (P3.121)
Conclusions: The burden of risk factors in our patients was very high. Although patients reported uniform adherence to treatment, risk factor control to target was good for hypertension and atrial fibrillation and poor for smoking, diabetes and dyslipidemia. Despite high rates of insurance and established primary care, multiple interventions were performed during this visit, particularly in the more disabled. The post-discharge SPC visit augments primary care and offers an important opportunity to review and revise the care plan, assess patient adherence and control of risk factors to target.*Chart and Graph imaging to be ...

Impact of Insurance Status on Outcomes and Use of Rehabilitation Services in Acute Ischemic Stroke: Findings From Get With The Guidelines-Stroke Health Services and Outcomes Research
The objective was to determine whether patients without insurance or with government‐sponsored insurance had worse quality of care or in‐hospital outcomes in acute ischemic stroke. Methods and ResultsMultivariable logistic regressions with generalized estimating equations stratified by age under or at least 65 years were adjusted for patient demographics and comorbidities, presenting factors, and hospital characteristics to determine differences in in‐hospital mortality and postdischarge destination. We included 589 320 ischemic stroke patients treated at 1604 US hospitals participating in the Get With The Guideline...

Racial differences in statin adherence following hospital discharge for ischemic stroke
Conclusions: Compared with white adults, black adults were more likely to be nonadherent to statins following hospitalization for ischemic stroke.

Ischemic Stroke in Nonvalvular Atrial Fibrillation at Warfarin Initiation Clinical Sciences
Conclusions—Warfarin effect was qualitatively different in the first 30 days after initiation than subsequently. This is consistent with a modest increase in stroke risk occurring briefly after starting warfarin.

Inequities in Telemedicine Use Among Patients With Stroke and Cerebrovascular Diseases: A Tricenter Cross-sectional Study
Discussion In our diverse cohort across 3 centers, we found differences in TM visit type by race and insurance early during the COVID-19 pandemic. These findings suggest disparities in VTM access across different stroke populations. As VTM remains an integral part of outpatient neurology practice, steps to ensure equitable access are essential.

Insurance Status and Outcome after Intracerebral Hemorrhage: Findings from Get With The Guidelines-Stroke
Backgound: Few studies have examined associations among insurance status, treatment, and outcomes in patients hospitalized for intracerebral hemorrhage (ICH).Methods: Through retrospective analyses of the Get With The Guidelines (GWTG)-Stroke database, a national prospective stroke registry, from April 2003 to April 2011, we identified 95,986 nontransferred subjects hospitalized with ICH. Insurance status was categorized as Private/Other, Medicaid, Medicare, or None/Not Documented (ND). Associations between insurance status and in-hospital outcomes and quality of care measures were analyzed using patient- and hospital-spec...

Disparities in Acute Stroke Severity, Outcomes, and Care Relative to Health Insurance Status
Background: To examine the differences in risk factors and length of hospital stay (LOS) between the insured and uninsured stroke patients, identifying the root causes of increasing hospital stay. Methods: Retrospective analysis of stroke registry data of acute stroke patients (N = 19,255) was analyzed to compare risk factors, severity, outcome, and LOS by insurance status. Chart review of patients from a comprehensive stroke center (N = 3290) was studied in greater detail for causes of extended length of stay. Results: The uninsured patients had poorer control of risk factors and statistically significantly (P

Socioeconomic Disparities in the Utilization of Mechanical Thrombectomy for Acute Ischemic Stroke
We examined the utilization rates of mechanical thrombectomy by race/ethnicity (white, black, Hispanic, and Asian/Pacific Islander), income quartile (first, second to third, and fourth), and insurance status (Medicare, Medicaid, self-pay, and private). We also studied thrombectomy utilization rates at hospitals that performed thrombectomy. Results: From 2006 to 2010, 2,087,017 patients were hospitalized with a primary diagnosis of acute ischemic stroke; 8946 patients (.4%) received mechanical thrombectomy. Compared with white patients, black patients had significantly lower rates of overall mechanical thrombectomy utilizati...

Gallstone Disease and the Risk of Stroke: A Nationwide Population-based Study
Background: Gallstone disease (GD) and stroke share a number of risk factors including diabetes and hyperlipidemia. This nationwide population-based study was designed to estimate the risk of stroke after a diagnosis of GD.Methods: Data were obtained from the Taiwan National Health Insurance Research Database. A total of 135,512 patients with a diagnosis of GD and 271,024 age- and gender-matched non-GD control patients were included to assess the risk of stroke using Cox proportional hazard regression. Results: During the study period (2000-2003), 12,234 (153.67/10,000 person-years) strokes occurred among the GD patients, a...

Association of Insurance Status with Stroke-Related Mortality and Long-term Survival after Stroke
Lack of insurance is a barrier to optimal stroke risk factor control but data on its long-term impact on stroke outcomes are sparse. We assessed the association between health insurance and long-term mortality after stroke.

Effect of Adherence to Oral Anticoagulants on Risk of Stroke and Major Bleeding Among Patients With Atrial Fibrillation Arrhythmia and Electrophysiology
Conclusions Adherence to anticoagulation is poor in practice and may be modestly improved with NOACs. Adherence to therapy appears to be most important in patients with CHA2DS2-VASc score ≥2, whereas the benefits of anticoagulation may not outweigh the harms in patients with CHA2DS2-VASc score 0 or 1.

Warfarin Use in Patients With Atrial Fibrillation Undergoing Hemodialysis Clinical Sciences
Conclusions—Our findings suggest that warfarin should be used carefully in hemodialysis patients, given the higher risk of hemorrhagic events and the lack of ability to prevent thromboembolic complications.

The Impact of Nurse Staffing on In-Hospital Mortality of Stroke Patients in Korea
Conclusion: Policies to educate sufficient numbers of nurses and retain them in the field are warranted, especially because medical-cost containment has become a dominant concern in most countries. Further studies are needed to understand the mechanisms and other protective roles of nurse staffing to ensure long-term health outcomes after hospital discharge.
