The Impact of a Stroke Prevention Clinic after Discharge for TIA/ Stroke: University Hospitals Case Medical Center Comprehensive Stroke Center Experience (P3.121)

Conclusions: The burden of risk factors in our patients was very high. Although patients reported uniform adherence to treatment, risk factor control to target was good for hypertension and atrial fibrillation and poor for smoking, diabetes and dyslipidemia. Despite high rates of insurance and established primary care, multiple interventions were performed during this visit, particularly in the more disabled. The post-discharge SPC visit augments primary care and offers an important opportunity to review and revise the care plan, assess patient adherence and control of risk factors to target.*Chart and Graph imaging to be added to Final Poster.
