Long term outcome of perinatal stroke.
Abstract Neonatal Arterial Ischemic Stroke (NAIS) affects 6-17 newborns on 100 000-birth term neonates, most of these children keeping long-term motor and cognitive impairments. Based on a literature review, the objectives of this paper are to describe motor and cognitive outcomes after a NAIS and to propose a consensual monitoring of these children to improve their management. About 30 % of children after a NAIS will develop a unilateral cerebral palsy requiring a management by a team with expertise in physical medicine and rehabilitation. Unlike adults, especially after a left NAIS, children will not present aph...

Neonatal arterial ischemic stroke: Which thrombotic biological risk factors to investigate and which practical consequences?
Abstract All biological risk factors that have been previously identified to increase the risk of thrombosis in adults, have also been studied in neonates with arterial Ischemic Stroke (NAIS), but most studies were retrospective and included relatively low numbers of affected children. We therefore could not suggest recommendations with a strong level of evidence and only expert proposals potentially useful for clinical practice will be presented in this text. Despite these limitations, the extensive analysis of published data supported that factor V Leiden (FVL) and increased levels of Lp(a) could be significant ...

Recommendations for imaging neonatal ischemic stroke.
Abstract Neuroimaging is critical for the diagnosis of neonatal arterial ischemic stroke (NAIS) and for prognosis estimation. The purpose of this work is to define guidelines of clinical neuroimaging for the diagnosis of NAIS, for the optimization of the imaging timing and for the assessment of the prognostic value of each imaging technique. A systematic search of electronic databases (Medline via Pubmed) for studies whose title and abstract were focused on NAIS has been conducted. One hundred and ten articles were selected and their results were analyzed by three Senior Practitioners of pediatric radiology using ...

Early predictors of neurodevelopment after perinatal arterial ischemic stroke: a systematic review and meta-analysis
CONCLUSION: Corticospinal tract assessment on MRI and standardized motor assessments are best to predict cerebral palsy after PAIS. Future research should be focused on improving outcome prediction for non-motor outcomes.IMPACT: We present a systematic review of early predictors for various long-term outcome categories after perinatal arterial ischemic stroke (PAIS), including a meta-analysis for the outcome unilateral spastic cerebral palsy. Corticospinal tract assessment on MRI and standardized motor assessments are best to predict cerebral palsy after PAIS, while bedside techniques such as (a)EEG and NIRS might improve ...

Incidence and Outcomes of Symptomatic Neonatal Arterial Ischemic Stroke
CONCLUSIONS: NAIS in Switzerland shows a similar incidence as other population-based studies. About one-third of patients developed cerebral palsy or showed delayed mental performance 2 years after birth, and children with normal mental performance may still develop deficits later in life.

Sydenham's chorea: not gone but perhaps forgotten
In this study, we investigated the incidence and clinical presentation of childhood SC in Ireland (years 2006–2014). Nineteen cases were diagnosed. Five patients had rheumatic fever. An increasing trend with an incidence of 0.23/100 000 is reported. As most referral diagnoses included psychogenic illness, head injury and stroke, modern physicians may not be aware of this age old illness. A review of the manifestations and diagnosis of SC is presented.

Stroke
This 18-month-old girl developed left-sided focal seizures, left arm and leg weakness 3 days after an uncomplicated appendicectomy. She had been previously well, and the surgery was uneventful. An urgent cranial MR scan was performed under general anaesthetic within 12 h of the onset of symptoms. Look at the selected images from this study and answer the following questions. Read on to confirm the answers and learn more about the use of diffusion-weighted imaging (DWI) in this condition. Questions There is evidence of acute intracerebral haemorrhage. (True or false?) The abnormality is in the left middle cerebral...

Abnormal neurodevelopmental outcomes are very likely in cases of bilateral neonatal arterial ischaemic stroke
ConclusionOur study showed that abnormal neurodevelopmental outcomes were very likely after cases of neonatal AIS with bilateral involvement and clinicians should consider early and more effective interventions in such cases. This article is protected by copyright. All rights reserved.

Neonatal Arterial Ischemic Stroke – A Hospital Based Active Surveillance Study in Germany
Conclusions NAIS is a rare but not negligible morbidity in newborns. Asymptomatic children account for 56% of NAIS in all neonates. In these, not only seizures but also other unexplained symptoms should trigger diagnostic work-up with cUS and cMRI. Negative initial ultrasound results do not exclude NAIS. Recognition, identification, and diagnosis announcement of neonatal arterial ischemic stroke: A combined exploratory quantitative and qualitative study on parents' lived experiences
CONCLUSION: Maternity hospital caregivers' awareness of NAIS is crucial to reach early diagnosis. Improving this aspect would not only allow better early management, but also make it possible to set up acute neuroprotective strategies. Clinicians should be attentive to the modalities of diagnosis and prognosis announcements, which are associated with considerable stress and misconceptions.PMID:33715932 | DOI:10.1016/j.arcped.2021.02.002
