“An aspirin a day keeps the doctor away,” an age-old adage revised to support stroke prevention, may no longer be relevant in light of new data presented at the European Society of Cardiology (ESC) Scientific Congress 2018 in Munich, Germany. Of course we have known for some time—and data cont inue to support us—that aspirin is highly effective in secondary prevention of cardiovascular disease (ie, in patients who have already had an adverse vascular event).

Atrial fibrillation
(AF) is the most common cardiac arrhythmia, with a high prevalence in elderly individuals. It is associated with increased mortality and morbidity, as a result of stroke, systemic embolism and heart failure. Stroke prevention is central in management of AF. AF management can be distilled into a simple integrated care approach – the ABC (Atrial fibrillation Better Care) pathway: (1) Avoid stroke, with anticoagulation. The risk of stroke in AF is related to increasing age and coexistent co-morbidities.

ConclusionsElectronic physician notification did not increase anticoagulation in patients with atrial fibrillation at elevated stroke risk. Patent foramen ovale closure or medical therapy for secondary prevention of cryptogenic stroke: An update meta-analysis of randomized controlled trials
Conclusion: PFO closure, as compared with medical therapy, was associated with decreased risk of recurrent stroke and increased risk of atrial fibrillation in cryptogenic stroke patients with PFO.

Prognostic value of von Willebrand factor in patients with atrial fibrillation: A meta-analysis
Conclusion: Patients with AF with elevated vWF level are independently associated with a higher risk of MACEs and all-cause mortality. However, more well-designed prospective studies are needed to confirm these findings.

Effects of perioperative statins on patient outcomes after noncardiac surgery: a meta-analysis.
CONCLUSIONS: This meta-analysis supports the hypothesis that perioperative statins effectively reduce the incidences of postoperative myocardial infarction, composite of death/myocardial infarction/stroke and new cases of atrial fibrillation in patients undergoing noncardiac surgery.
