The objective of this review was to identify causes of stroke/death after carotid endarterectomy (CEA) and to develop transferable strategies for preventing stroke/death after CEA, via an overview of a 21-year series of themed research and audit projects. Discussion Stroke is a devastating neurologic condition with an alarming prevalence. Each year, an estimated 795,000 people in the United States alone will suffer a stroke. Stroke accounts for one in every 20 deaths in the U.S., and someone dies of stroke in the U.S. every four minutes.1 One third of people who have had a stroke will be left with some degree of long-term disability.2 Eighty-seven percent of all strokes are ischemic, meaning that a clot or other occlusion to blood flow forms within an intracranial vessel, depriving the brain tissue of blood flow.1 If this obstruction isn't rapidly relieved, damage to the brain will occur.

Cyclophilin A: A predictive biomarker of carotid stenosis in cerebral ischemic stroke.

Intracranial Large Artery Disease is Independently Associated with Poor Functional Outcome in a Cohort of Ethnic South Asian Ischemic Stroke Patients
Conclusions: The presence of ICLAD rendered poorer functional prognosis after stroke. These findings support the specific evaluation of the benefits of known acute stroke treatments such as thrombolysis, as well as investigation of potential novel strategies such as acute stenting.

Carotid artery stenosis, an underestimated cause of stroke recurrence in patients with ischaemic monocular visual loss.
CONCLUSIONS: Carotid stenosis ≥ 50% is present in patients with ocular ischaemia in approximately 20% of those with persistent visual loss and in 10% with transient visual loss. Those with carotid stenosis have a higher risk of stroke recurrence and should be considered urgent surgical intervention as other forms of stroke. PMID: 31155910 [PubMed - as supplied by publisher]

Prospective Study About the Relationship Between CEUS of Carotid Intraplaque Neovascularization and Ischemic Stroke in TIA Patients
Conclusion For carotid plaques, CEUS could evaluate the infusion mode of them, which could reflect the neovascularization in plaques. CEUS could predict the incurrence of ischemic stroke or recurrence of TIA in TIA patients, which was useful information for clinical decision.

Using Ultrasound and Inflammation to Improve Prediction of Ischemic Stroke: A Secondary Analysis of the Multi-Ethnic Study of Atherosclerosis
Conclusions: In a multiethnic cohort of patients without cerebrovascular disease at baseline, we found that CD, CS, and IL-6 helped predict the occurrence of primary ischemic stroke.
