Rare stroke mechanisms in 4154 consecutive patients: causes, predictors, treatment, and outcomes
CONCLUSIONS: RMS occurred in 5.3% of a large population of consecutive AISs and are most frequently related to medical interventions, cancer, and vasculitis. RMS patients have less traditional risk factors but more systemic comorbidities, hemorrhagic transformations, recurrences, and a worse long-term outcome. CONCLUSIONS:Persistent IONM changes had a strong correlation with new post-operative deficits. Transient IONM changes, including those resolving after alterations in surgical management or pre-embolization methohexital testing, had a much lower risk of new deficits. A stair-climbing test for measuring mechanical efficiency of ambulation in adults with chronic stroke.
Conclusion: With a minimal detectable change of 0.6% (0.053 of average MEnet score of 10.4%), MEnet values from the stair-climbing test seem sufficiently meaningful to estimate ambulatory ability and its changes with interventions or walking aids in adults with hemiparesis. Implications for Rehabilitation Ambulatory ability can be estimated from mechanical efficiency, obtained from a 5-min stair-climbing test utilizing a 4-step stair, to measure external work, and the change in heart rate above rest to estimate the metabolic cost of the task. ConclusionThe psychometric properties of the measurement tool showed acceptable results allowing its implementation in clinical practice by the nursing community staff for evaluating practical skills in informal caregivers when providing care to older stroke survivors living at home.

How physical exercise aids in stroke recovery
(Frontiers) Study shoes that mice that had free-access to a running wheel were able to maintain ocular dominance plasticity after suffering a stroke, compared to those that didn't. These exciting observations have the potential to provide a simple but effective method to protect and rehabilitate patients that are prone to, or have already suffered, a stroke.

The effect of an arm sling used for shoulder support on gait efficiency in hemiplegic patients with stroke using walking aids-a randomized clinical trial.
CONCLUSIONS: Among the hemiplegic patients with a single cane, the walking endurance achieved with an arm sling significantly improved than those achieved without an arm sling, and the energy expenditure and energy cost was significantly lower.
CLINICAL REHABILITATION IMPACT: The hemiplegic arm support with an arm sling may be beneficial for gait efficiency in hemiplegic patients using a single cane, which lead to decreased oxygen use at a given speed.

People with acquired immune deficiency syndrome (AIDS) develop ischemic stroke through distinct mechanisms. These include infections such as syphilis, tuberculosis, varicella, and other conditions such as cocaine abuse, endocarditis, and hypercoagulability. The effect of improved awareness, detection, and treatment with highly active antiretroviral therapy (HAART) on the incidence and outcome of AIDS patients with stroke is unknown.

Conclusion Cwfree was strongly correlated with Sfree, which allowed for the development of a valid Cwfree prediction model. A practitioner could estimate the energy expenditure of walking for a patient without using an indirect calorimeter.
