Drug effects on the cardiovascular system in conscious rats – separating cardiac output into heart rate and stroke volume using PKPD modeling
Conclusions and ImplicationsA systems pharmacology model characterizing the interrelationship between MAP, CO, HR, SV and TPR has been obtained in hypertensive and normotensive rats. The extended model can be used to quantify the dynamic changes in the CVS and elucidate the MoA for novel compounds, with one site of action, using HR and MAP measurements only. The question whether the model can also be applied for compounds with a more complex mechanism of action remains to be established.

Blood pressure control and cardiovascular outcomes in normal-weight, overweight, and obese hypertensive patients treated with three different antihypertensives in ALLHAT
Objective:Epidemiologically, there is a strong relationship between BMI and blood pressure (BP) levels. We prospectively examined randomization to first-step chlorthalidone, a thiazide-type diuretic; amlodipine, a calcium-channel blocker; and lisinopril, an angiotensin-converting enzyme inhibitor, on BP control and cardiovascular outcomes in a hypertensive cohort stratified by baseline BMI [kg/m2; normal weight (BMI 30)]. Methods:In a randomized, double-blind, practice-based Antihypertensive and Lipid-Lowering Treatment to Prevent Heart Attack Trial, 33 357 hypertensive participants, aged at least 55 years, were followed...

Amlodipine and cardiovascular outcomes in hypertensive patients: meta-analysis comparing amlodipine-based versus other antihypertensive therapy.
CONCLUSIONS: Amlodipine reduced the risk of total cardiovascular events as well as all-cause mortality compared with non-CCB-based regimens, indicating its benefit for high-risk cardiac patients. PMID: 24851066 [PubMed - in process]

Prolonged anticoagulant activity of rivaroxaban in a polymorbid elderly female with non-convulsive epileptic state
Conclusion: Health care providers should consider renal function, concomitant medication, polymorbidity and age prior to prescribing rivaroxaban. Care has to be taken when prescribing rivaroxaban to patients who are different from those included in the ROCKET AF trial.

The Value of N-Terminal Pro-B-Type Natriuretic Peptide in Determining Antihypertensive Benefit: Observations From the Anglo-Scandinavian Cardiac Outcomes Trial (ASCOT) Natriuretic Peptides
We investigated 3 hypotheses: (1) N-terminal pro–B-type natriuretic peptide (NT-proBNP) predicts cardiovascular disease events in patients with hypertension, (2) NT-proBNP is associated with blood pressure variability, and (3) NT-proBNP predicts benefit from antihypertensive regimens. The Anglo-Scandinavian Cardiac Outcomes Trial (ASCOT) randomized a subset of 6549 patients at risk with no history of coronary heart disease to either atenolol-based or amlodipine-based blood pressure–lowering treatment. During 5.5 years of follow-up, 485 cardiovascular disease cases accrued and were matched with 1367 controls. Ba...

Long-term prehypertension treatment with losartan effectively prevents brain damage and stroke in stroke-prone spontaneously hypertensive rats.
Abstract Prehypertension has been associated with adverse cerebrovascular events and brain damage. The aims of this study were to investigate ⅰ) whether short‑ and long-term treatments with losartan or amlodipine for prehypertension were able to prevent blood pressure (BP)-linked brain damage, and ⅱ) whether there is a difference in the effectiveness of treatment with losartan and amlodipine in protecting BP-linked brain damage. In the present study, prehypertensive treatment with losartan and amlodipine (6 and 16 weeks treatment with each drug) was performed on 4-week‑old stroke-prone spontaneously hypert...

Comparison of the Effects of Amlodipine and Losartan on Blood Pressure and Diurnal Variation in Hypertensive Stroke Patients: A Prospective, Randomized, Double-Blind, Comparative Parallel Study.
CONCLUSIONS: The noninferiority of amlodipine was not confirmed by the per-protocol analysis. However, amlodipine showed a favorable effect on the morning surge. ClinicalTrials.gov identifier: NCT01830517. PMID: 24296324 [PubMed - as supplied by publisher]

Advantages of ramipril/amlodipin fixed combination therapy. When should we use it?.
Abstract Hypertension is one of the most frequent chronic disease in Hungary and one of the most important cardiovascular risk factor. Treatment of blood pressure to target value lowers significantly the risk of coronary artery disease, stroke and chronic renal disease as well as it decreases mortality. Blood pressure control has a great importance in high risk patients. In addition to life style changes, drug treatment plays an essential role in the management of hypertensive patients. The complexity of drug regimen, characteristics of drug class, age and gender are all exert impacts on patient adherence. Antihyp...

Effects of Valsartan Versus Amlodipine in Diabetic Hypertensive Patients With or Without Previous Cardiovascular Disease
In conclusion, the ARB- and the CCB-based treatments exerted similar protective effects of CVD events regardless of the presence of previous CVD. For stroke events, the ARB may have more protective effects than the CCB in diabetic hypertensive patients with previous CVD.

Results from A 12 Months, Randomized, Clinical Trial Comparing an Olmesartan/Amlodipine Single Pill Combination to Olmesartan and Amlodipine Monotherapies on Blood Pressure and Inflammation.
CONCLUSION: other than to be more effective in reducing blood pressure, olmesartan/amlodipine single pill combination gave also a major increase of insulin sensitivity and a decrease of inflammatory markers compared to single monotherapies. PMID: 23999037 [PubMed - as supplied by publisher]

Mortality and Morbidity During and After Antihypertensive and Lipid-Lowering Treatment to Prevent Heart Attack Trial: Results by Sex Clinical Trial - ALLHAT
To determine whether an angiotensin-converting enzyme inhibitor (lisinopril) or calcium channel blocker (amlodipine) is superior to a diuretic (chlorthalidone) in reducing cardiovascular disease incidence in sex subgroups, we carried out a prespecified subgroup analysis of 15 638 women and 17 719 men in the Antihypertensive and Lipid-Lowering Treatment to Prevent Heart Attack Trial (ALLHAT). Total follow-up (active treatment + passive surveillance using national administrative databases to ascertain deaths and hospitalizations) was 8 to 13 years. The primary outcome was fatal coronary heart disease or nonfatal myocardial i...

Is this Dementia or Alcohol Intoxication or Both?
I am presenting the case of an independent lady whose active alcohol abuse masked her new onset dementia. Ms. G is a 76 year old female who lives alone in a continuum of care retirement community. She has difficult-to-control hypertension, hypothyroidism, B12 deficiency, anxiety, and adjustment disorder which started after her husband, who was an alcoholic, committed suicide. She struggled with alcohol abuse herself through the years. She takes citalopram, amlodipine, B12 injections, and vitamin D supplements. She has lived in this community for 4 years. She has made new friends and currently has a new boyfriend, with who...
