Identification, characterization, and engineering of glycosylation in thrombolyticsa
Biotechnol Adv. 2023 May 12:108174. doi: 10.1016/j.biotechadv.2023.108174. Online ahead of print.ABSTRACTCardiovascular diseases, such as myocardial infarction, ischemic stroke, and pulmonary embolism, are the most common causes of disability and death worldwide. Blood clot hydrolysis by thrombolytic enzymes and thrombectomy are key clinical interventions. The most widely used thrombolytic enzyme is alteplase, which has been used in clinical practice since 1986. Another clinically used thrombolytic protein is tenecteplase, which has modified epitopes and engineered glycosylation sites, suggesting that carbohydrate modifica...

Effect of Intravenous Thrombolytic Dose of Alteplase on Long-Term Prognosis in Patients with Acute Ischemic Stroke
ConclusionsStandard-dose alteplase treatment reduced the risk of long-term recurrence of AIS after hospital discharge and the alteplase dose was not associated with the long-term risk of death from AIS.

Effect of alteplase, benzodiazepines and beta-blocker on post-stroke pneumonia: Exploration of VISTA-Acute
by Thanh G. Phan, Richard Beare, Philip M. Bath, Svitlana Ievlieva, Stella Ho, John Ly, Amanda G. Thrift, Velandai K. Srikanth, Henry Ma, on behalf of the VISTA-Acute Collaborators BackgroundPost-stroke pneumonia is a frequent complication of stroke and is associated with high mortality. Investigators have described its associations with beta-blocker. However, there has been no evaluation of the role of recombinant tissue plasminogen activator (RTPA). We postulate that RTPA may modify the effect of stroke on pneumonia by reducing stroke disability. We explore this using data from neuroprotection trials in Virtual Internati...

Thrombectomy with or without Bridging Thrombolysis for Anterior Circulation Stroke
Conclusions: In this study, BT seems to yield better clinical and procedural outcomes with lower complication rates than d-MT. These findings may support the additional value of intravenous alteplase in anterior system strokes. Further large-scale, prospective, randomized-controlled studies will clarify the gray lines in this consensus, but this paper is important for reflecting the real-world data in developing countries.

Low-dose vs. standard-dose intravenous alteplase for acute ischemic stroke with unknown time of onset
ConclusionThe effectiveness of low-dose alteplase might be comparable to that of standard-dose alteplase in AIS patients aged <70 years with favorable perfusion-imaging profiles in the unknown or extended time window but not in those aged ≥70 years. Furthermore, low-dose alteplase did not significantly reduce the risk of sICH compared to standard-dose alteplase.

Follow-up Imaging After Thrombolysis: FIAT, A Randomized Trial
Current protocols for treatment of acute ischemic stroke with intravenous thrombolytics, such as alteplase (tPA) and tenecteplase (tNK), recommend the completion of a routine non-contrast head CT at 24 hours post treatment to evaluate for hemorrhage prior to the initiation of antiplatelet therapy for secondary stroke prevention. This guideline was instituted because it had been part of the protocol in the NINDS multicenter randomized placebo-controlled trial that showed the benefit of IV thrombolytics within 3 hours of stroke onset.

Early recanalization with intravenous thrombolysis before mechanical thrombectomy: considerations to explore with tenecteplase
Time is brain. Hence, timely reperfusion in emergent large vessel occlusion (LVO) stroke is critical for favorable outcomes. Intravenous thrombolysis with alteplase (TPA) before mechanical thrombectomy (MT) for emergent LVO stroke is associated with significantly higher rates of preinterventional reperfusion than MT alone. Recent post hoc analyses of SWIFT-DIRECT and DIRECT-MT have shown that times longer than 30 min since TPA administration are associated with increased reperfusion rates.1 2 The incremental increase of reperfusion rates with longer times since TPA administration underpins TPA’s time dependence to ex...

Biosimilar Tenecteplase Versus Alteplase in Acute Ischemic Stroke: A Real World Study
CONCLUSION: This present study observed no difference in the efficacy and safety between biosimilar TNK and rTPA. Our findings are in concordance with published trials showing equivalence between the two molecules.PMID:37034051 | PMC:PMC10081554 | DOI:10.4103/aian.aian_754_22

Infarct Evolution in Patients with Anterior Circulation Large-Vessel Occlusion Randomized to IV Alteplase and Endovascular Treatment versus Endovascular Treatment Alone INTERVENTIONAL
CONCLUSIONS: No statistically significant difference in infarct evolution was found in directly admitted patients who received IV alteplase and endovascular treatment within 4.5 hours of symptom onset versus patients who underwent endovascular treatment alone. Collateral status and occlusion location may be useful predictors of infarct evolution prognosis in patients eligible for IV alteplase who underwent endovascular treatment.

Impact of emergency department arrival time on door-to-needle time in patients with acute stroke
ConclusionFactors that contribute most to a delay in DTN time include arrival during the night shift, lack of a dedicated stroke team, longer time-to-CT read, and arrival as a walk-in. All of these are addressable factors from an operational standpoint and should be considered when performing quality improvement of hospital protocols.
