Current knowledge on assessing the effects of and managing bleeding and urgent procedures with direct oral anticoagulants.
CONCLUSION: With or without reversal agents, patients may require supportive management such as mechanical pressure, volume support, transfusions of blood products, and, depending on the situation, surgery to repair the bleeding source. Specific reversal agents are currently under development or have recently been approved for the urgent management of bleeding events or the facilitation of invasive procedures in patients receiving DOACs. A Case of Transient Global Amnesia: A Review and How It May Shed Further Insight into the Neurobiology of Delusions
Conclusion In closing, our patient’s episode of TGA combined with her emotional and perceptual response lends credence to the proposal of a “fear/paranoia” circuit in the genesis of paranoid delusions—a circuit incorporating amygdala, frontal, and parietal cortices. Here, neutral or irrelevant stimuli, thoughts, and percepts come to engender fear and anxiety, while dysfunction in frontoparietal circuitry engenders inappropriate social predictions and maladaptive inferences about the intentions of others.[54] Hippocampus relays information about contextual information based on past experiences and the current situat...

Clinical features and prognostic value of stent-graft-induced post-implantation syndrome after thoracic endovascular aortic repair in patients with type B acute aortic syndromes AORTIC SURGERY
CONCLUSIONS PIS occurred in 15.8% of patients with AASs without affecting in-hospital outcome. At follow-up, PIS was associated with increased rates of MAEs, but not mortality.

Hemiarch replacement with concomitant antegrade stent grafting of the descending thoracic aorta versus total arch replacement for treatment of acute DeBakey I aortic dissection with arch tear AORTIC SURGERY
CONCLUSION In treating DeBakey I aortic dissection with arch tear, hemiarch replacement with primary tear repair and concomitant TEVAR is a safe alternative to conventional TAR, with improved distal aortic remodelling.

Long-term Outcome After EndoVAC Hybrid Repair of Infected Vascular Reconstructions.
CONCLUSION: The EndoVAC technique is an alternative, less invasive, option for treatment of infected vascular reconstructions in selected cases, when neither traditional radical surgery, nor conservative simple negative pressure wound therapy are considered feasible or safe. The exact indications for this alternative hybrid treatment need to be established. PMID: 26944600 [PubMed - as supplied by publisher]

This Vitamin Can Save Your Brain
Your brain has amazing abilities. And it can heal itself. That’s not something you’re likely to hear from mainstream medicine — especially if you or a loved one suffer from the effects of stroke, Alzheimer’s, Parkinson’s or another form of dementia. Here at the Sears Institute for Anti-Aging Medicine, I’ve seen many people with brain damage. And, sadly, most have been deemed “hopeless” by so-called medical experts. But I can tell you there is nothing hopeless about dementia — no matter what its cause. With the right nutrients, there are times when brain damage can be reversed. And now recent research from...

Evaluation of the downstream aorta after frozen elephant trunk repair for aortic dissections in terms of diameter and false lumen status AORTIC SURGERY
CONCLUSIONS Compared with conventional surgery for extensive ADs, the FET technique provides a high rate of FL thrombosis of the thoracic aorta.

How to define valvular atrial fibrillation?
Abstract Atrial fibrillation (AF) confers a substantial risk of stroke. Recent trials comparing vitamin K antagonists (VKAs) with non-vitamin K antagonist oral anticoagulants (NOACs) in AF were performed among patients with so-called "non-valvular" AF. The distinction between "valvular" and "non-valvular" AF remains a matter of debate. Currently, "valvular AF" refers to patients with mitral stenosis or artificial heart valves (and valve repair in North American guidelines only), and should be treated with VKAs. Valvular heart diseases, such as mitral regurgitation, aortic stenosis (AS) and aortic insufficiency, do...

Long-Term Outcomes of the Maze Procedure Combined With Mitral Valve Repair: Risk of Thromboembolism Without Anticoagulation Therapy.
CONCLUSIONS: Adopting appropriate selection criteria for discontinuing anticoagulation therapy, the majority of patients receiving concomitant mitral repair and the Maze procedure can discontinue warfarin with excellent long-term safety profiles. PMID: 26116476 [PubMed - as supplied by publisher]

Gap junctions and connexin hemichannels in the regulation of haemostasis and thrombosis.
Abstract Platelets are involved in the maintenance of haemostasis but their inappropriate activation leads to thrombosis, a principal trigger for heart attack and ischaemic stroke. Although platelets circulate in isolation, upon activation they accumulate or aggregate together to form a thrombus, where they function in a co-ordinated manner to prevent loss of blood and control wound repair. Previous report (1) indicates that the stability and functions of a thrombus are maintained through sustained, contact-dependent signalling between platelets. Given the role of gap junctions in the co-ordination of tissue respo...

Intraoperative Sonography During Carotid Endarterectomy: Normal Appearance and Spectrum of Complications
Carotid endarterectomy is a commonly performed procedure for prevention of stroke related to carotid stenosis. Intraoperative sonography is used to identify potentially correctable technical defects during carotid endarterectomy. The main risk of endarterectomy is perioperative stroke, and great effort has been put into trying to reduce this risk through various surgical techniques and evaluation of the surgical bed. Postoperative carotid thrombosis, or thombo-embolization from the arterectomy site, remains a common cause of perioperative stroke and is often related to technical defects in the arterial reconstruction proce...

Should TCDs be routinely used for evaluating intracranial circulation for screening high risk patients prior to cardiac surgery? (P2.280)
CONCLUSIONS: Our case elucidates the possibility of a hypoperfusion stroke related to left vertebral dominantly supplying intracranial circulation in presence of bilateral significant carotid stenosis. Due to lack of previous cardio/cerebrovascular events, his carotid stenosis was deemed asymptomatic prior to cardiac surgery. We propose that intracranial collateral flow should be investigated via TCD in patients with bilateral carotid stenosis undergoing procedures with high stroke risk to assess safety and efficacy of carotid revascularization in this small but significant subset. Additional studies like acetazolamide wit...

The frozen elephant trunk technique for the treatment of complicated type B aortic dissection with involvement of the aortic arch: multicentre early experience AORTIC SURGERY
CONCLUSION The FET technique is a feasible therapeutic option for complicated type B AD with involvement of the aortic arch if TEVAR is contraindicated. In contrast to conventional aortic surgery via a lateral thoracotomy, the FET procedure can provide simultaneous treatment of the ascending aorta and aortic arch.
