Asymptomatic Carotid Stenosis Is Associated With Circadian and Other Variability in Embolus Detection
Conclusions: Embolism associated with asymptomatic carotid stenosis shows circadian variation with highest rates 4–6 h before midday. This corresponds with peak circadian incidence of stroke and other vascular complications. These and ASED Study results show that monitoring frequency, duration, and time of day are important in ES detection. Introduction Transcranial Doppler (TCD) detected microembolism in the ipsilateral middle cerebral artery (MCA) may help stratify the risk of stroke and other arterial disease complications in persons with advanced (≥60%) asymptomatic carotid stenosis. If so, this t...

Cryptotanshinone Attenuates Oxygen-Glucose Deprivation/ Recovery-Induced Injury in an in vitro Model of Neurovascular Unit
Conclusions Despite the above limitations, we indicate that the protective mechanism of CTs against OGD/R damage might exert via inhibiting neuron apoptosis and attenuating BBB disruption. Furthermore, we also clarified that CTs inhibited neuronal apoptosis possibly by blocking the activation of MAPK signaling pathways, and CTs alleviating BBB disruption may associated with the regulation of TJPs and MMP-9 in our experiment. Accordingly, CTs will represent a novel and potent candidate for the treatment of CIRI in the future. Ethics Statement This study was carried out in accordance with the recommendations of China...

Factors influencing nonadministration of thrombolytic therapy in early arrival strokes in a university hospital in Hyderabad, India
Conclusion: One-fourth of early ischemic stroke patients in our study were not thrombolyzed even though they arrived within the window period. The majority of the reasons for nonadministration of thrombolysis were potentially preventable, such as nonaffordability, intrahospital delay, and nonavailability of newer endovascular interventions.

Direct oral anticoagulants in treatment of cerebral venous thrombosis: a systematic review protocol
DiscussionCurrent guidelines in the treatment of cerebral vein thrombosis dating back to 2011 from the American Heart Association/American Stroke Association endorse the utility of anticoagulation for the treatment of cerebral vein thrombosis; however, they did not support the use of direct oral anticoagulants. Updated guidelines from the European Stroke Organization, endorsed by the European Academy of Neurology in 2017, also refute utilization of direct oral anticoagulants due to a lack of evidence. There have been nearly 10  years of experience with direct oral anticoagulants in the treatment of venous thrombosis and ...

Predisposing factors and management of hemodynamic depression following carotid artery stenting
Pachipala Sudheer, Ayush Agarwal, Venugopalan Y Vishnu, MV Padma SrivastavaAnnals of Indian Academy of Neurology 2021 24(3):315-318 Carotid artery stenting (CAS) involves dilatation of carotid bulb which can trigger a series of neuronal responses resulting in hemodynamic depression that might influence the outcome of the procedure. This is a frequent but underdiagnosed complication of CAS. Although it is mild, transient and self-limiting in majority of cases, it can result in significant morbidity and mortality if persistent. Neurologists should be aware of the predisposing factors and management of this common complicati...

VWF linkage and GWAS Genetics
The plasma glycoprotein von Willebrand factor (VWF) exhibits fivefold antigen level variation across the normal human population determined by both genetic and environmental factors. Low levels of VWF are associated with bleeding and elevated levels with increased risk for thrombosis, myocardial infarction, and stroke. To identify additional genetic determinants of...

Rivaroxaban: a novel oral anticoagulant for the prevention and treatment of several thrombosis‐mediated conditions
The development of rivaroxaban (XARELTO®) is an important new medical advance in the field of oral anticoagulation. Thrombosis‐mediated conditions constitute a major burden for patients, healthcare systems, and society. For more than 60 years, the prevention and treatment of these conditions have been dominated by oral vitamin K antagonists (such as warfarin) and the injectable heparins. Thrombosis can lead to several conditions, including deep vein thrombosis, pulmonary embolism, myocardial infarction, stroke, and/or death. Prevention and treatment of thrombosis with an effective, convenient‐to‐use oral anticoagula...

Anticoagulation needs in asians with atrial fibrillation: a mythbuster.
Abstract Atrial fibrillation (AF) has long been known to increase the risk of stroke. As a result, relevant international guidelines recommend that measures to reduce the risk of thrombus formation should be considered in all patients presenting with AF. Based on risk assessment scores, patients would then receive either thrombocyte aggregation inhibitors or oral anticoagulants. Despite this advice, compliance rates with the recommendations are poor across all countries surveyed. Evidence from the Global Anticoagulant Registry in the Field (GARFIELD) registry shows that major deviation from guidelines is due in la...

A thrombus in transit through a patent foramen ovale
ABSTRACT: Patent foramen ovale (PFO) is a congenital heart defect that may first be diagnosed in adulthood and has a prevalence of 25% to 30%. Although many patients with PFO are asymptomatic and do not require treatment, paradoxical embolism can cause stroke or myocardial infarction. The authors report an unusual case of PFO with a transversing thrombus in an 80-year-old man. The patient's initial presentation appeared clinically as acute coronary syndrome, but he was subsequently diagnosed with a massive thrombus in transit via a PFO and pulmonary embolus leading to right-sided heart failure.

Crohns disease with central nervous system vasculitis causing subarachnoid hemorrhage due to aneurysm and cerebral ischemic stroke
We report a 39-year-old male who presented with headache, vomiting, and left-sided weakness in the known case of CD. Cross-sectional imaging (computed tomography and magnetic resonance imaging,) showed right gangliocapsular acute infarct with supraclinoid cistern subarachnoid hemorrhage (SAH). Cerebral digital substraction angiography (DSA) showed dilatation and narrowing of right distal internal carotid artery (ICA). Left ICA was chronically occluded. His inflammatory markers were significantly raised. Imaging features are suggestive of cerebral vasculitis. Arterial and venous infarcts due to thrombosis are known in CD. O...

Preventing Venous Thromboembolism in the Ambulatory Surgical Setting
Venous thromboembolism (VTE) is the third most common cardiovascular illness after acute coronary syndrome and stroke.1-3 Although the exact incidence of VTE is unknown, there are approximately one million cases in the United States each year, many of which are recurrent episodes.3 Nearly two-thirds of all VTE events result from hospitalization, and approximately 300,000 of these patients die.3 There are few statistics on VTE in the ambulatory setting; according to an American Academy of Orthopaedic Surgeons report, “Knee arthroscopy is the most commonly performed orthopaedic procedure in the United States.

Arguments favoring low versus high dose aspirin in the prophylaxis of venous thromboembolism
The use of aspirin in the prevention of venous thromboembolism (VTE) is still controversial. In a profound review on the use of aspirin for primary and secondary prevention of venous thromboembolism and other cardiovascular disorders Cohen et al. [1] conclude that the benefits of aspirin are well documented for conditions like myocardial infarction, coronary heart disease, and stroke, but less clearly for prevention of VTE after orthopedic surgery. The latter indistinctness has been a matter of concern in many earlier reviews and meta-analyses, and has even led to non-uniform guidelines on VTE prevention from the American ...
