Uncomon Cause Stroke: Thalamic Stroke As Initial Presentation Of Dunnigan Familial Partial Lipodystrophy (FPLD) (P6.264)
We report a laminopathy case with no history of MS suffering strokeREPORT/CASEA Woman of 40 years old came to the stroke unit because of tingling in her right arm with acute onset. She suffers miscarriage twice. Her familial history reports a pseudocushingoid facies in a sister, a brother, some paternal uncles and her small daughter. She had no history of hypertension or diabetes.Neurological and general examination found pseudocushingoid facies, faciotroncular obesity, hirsutism, muscular hyperthrophy on upper body but amyotrophy of the lower part, proeminentes venous, a little adiposus tissue, acanthosis migrans on the n...

Nafamostat Mesilate Improves Function Recovery after Stroke by Inhibiting Neuroinflammation in Rats
Publication date: Available online 24 March 2016 Source:Brain, Behavior, and Immunity Author(s): Chenhui Li, Jing Wang, Yinquan Fang, Yuan Liu, Tao Chen, Hao Sun, Xin-Fu Zhou, Hong Liao Inflammation plays an important role in stroke pathology, making it a promising target for stroke intervention. Nafamostat mesilate (NM), a wide-spectrum serine protease inhibitor, is commonly used for treating inflammatory diseases, such as pancreatitis. However, its effect on neuroinflammation after stroke was unknown. Hence, the effects of NM on the inflammatory response post stroke were characterized. After transient middle ce...

Hypertriglyceridemia may contribute to stroke and pancreatitis: A case report and review of the literature
We present a case of hypertriglyceridemia-associated stroke and pancreatitis in a 39-year-old woman. The patient’s laboratory tests reported high triglyceride concentrations beyond the instrument’s detection range, and radiological examination showed typical signs of cerebral infarction and acute pancreatitis. The patient received combined blood purification therapy, intravenous thrombolysis with urokinase, and conservative treatment of pancreatitis. We discuss the clinical features, pathogenesis, diagnosis, and treatment of hypertriglyceridemic stroke and pancreatitis combined with the relevant literature. We reviewed...

Management considerations for stroke-like episodes in MELAS with concurrent COVID-19 infection
We present a challenging case of an adult patient with a known diagnosis of Mitochondrial Encephalomyopathy, Lactic Acidosis and Stroke-like Episodes (MELAS) complicated by COVID-19 infection. She initially presented with altered mental status and vomiting and went on to develop a stroke-like episode, pancreatitis, and pneumatosis intestinalis. We review salient features of her hospitalization, including initiation of thromboprophylaxis in relation to intravenous arginine therapy, caution regarding medications such as remdesivir, and the incidence of gastrointestinal complications.

Ischemic Stroke in a Child after a Probable Scorpion Sting
We report a 2-year-old Venezuelan boy presenting with acute pancreatitis and pulmonary edema without an identifiable cause 48 hours after his initial symptoms. We administered antivenom therapy when an undetected scorpion sting was suspected. Despite some initial clinical improvement with respect to his acute pancreatitis, pulmonary edema, and coagulation abnormalities, our patient experienced an ischemic stroke. Fortunately, our patient did demonstrate some neurological improvement. Although acute pancreatitis and pulmonary edema are known end-organ damage manifestations of the sting of Tityus in the Americas, our particu...

Dipeptidyl peptidase-4 inhibitors, glucagon-like peptide 1 receptor agonists and sodium-glucose co-transporter-2 inhibitors for people with cardiovascular disease: a network meta-analysis
CONCLUSIONS: Findings from both standard and network meta-analyses of moderate- to high-certainty evidence suggest that GLP-1RA and SGLT2i are likely to reduce the risk of CVD mortality and all-cause mortality in people with established CVD; high-certainty evidence demonstrates that treatment with SGLT2i reduce the risk of hospitalisation for HF, while moderate-certainty evidence likely supports the use of GLP-1RA to reduce fatal and non-fatal stroke. Future studies conducted in the non-diabetic CVD population will reveal the mechanisms behind how these agents improve clinical outcomes irrespective of their glucose-lowerin...

Individualized Early Goal-Directed Therapy in Systemic Inflammation: Is Full Utilization of Preload Reserve the Optimal Strategy?
Objectives: In severe acute pancreatitis, the administration of fluids in the presence of positive fluid responsiveness is associated with better outcome when compared to guiding therapy on central venous pressure. We compared the effects of such consequent maximization of stroke volume index with a regime using individual values of stroke volume index assessed prior to severe acute pancreatitis induction as therapeutic hemodynamic goals. Design: Prospective, randomized animal study. Setting: University animal research laboratory. Subjects: Thirty domestic pigs. Interventions: After randomization, fluid resuscitation was s...

Effects of glucagon-like peptide-1 receptor agonists on major cardiovascular events in patients with Type 2 diabetes mellitus with or without established cardiovascular disease: a meta-analysis of randomized controlled trials
Conclusion  Glucagon-like peptide-1 receptor agonists significantly reduce MACE, CV and total mortality stroke, and hospitalization for HF, with a trend for reduction of MI, in patients with Type 2 DM with and without established CVD.

Alcohol and the heart: To abstain or not to abstain?
Abstract: Alcohol has been consumed by most societies over the last 7000years. Abraham Lincoln said “It has long been recognized that the problems with alcohol relate not to the use of a bad thing, but to the abuse of a good thing.” Light to moderate alcohol consumption reduces the incidence of coronary heart disease (CHD), ischemic stroke, peripheral arterial disease, CHD mortality, and all-cause mortality, especially in the western populations. However, heavy alcohol consumption is detrimental causing cardiomyopathy, cardiac arrhythmias, hepatic cirrhosis, pancreatitis, and hemorrhagic stroke. In this article, we rev...

Regional susceptibility to stress-induced intestinal injury in the mouse.
Abstract Injury to the intestinal mucosa is a life-threatening problem in a variety of clinical disorders, including hemorrhagic shock, trauma, burn, pancreatitis and heat stroke. However, the susceptibility to injury of different regions of intestine in these disorders is not well understood. We compared histological injury across the small intestine in two in vivo mouse models of injury, hemorrhagic shock (30% loss of blood volume) and heat stroke (peak core temperature,42.4°C). In both, areas near the duodenum showed significantly greater mucosal injury and reductions in villi height. To determine if these eff...
