Thrombectomy with and without computed tomography perfusion imaging for large-vessel occlusion stroke in the extended time window: a meta-analysis of randomized clinical trials
ConclusionThese findings suggest that the choice of NCCT ± CTA (without CTP) for the assessment of mechanical thrombectomy within 6–24 h after LVO in the anterior circulation is not significantly different from CTP; instead, the choice of NCCT ± CTA significantly reduces the time from onset to arterial puncture.

Analysis of Risk Factors for Epilepsy after Combined Re-Vascularization in Adult Patients with Moyamoya Disease
Conclusions: Pre-operative epilepsy, the location of the bypass recipient artery, new cerebral infarction, hyper-perfusion syndrome, and intra-cranial hemorrhage may have a causal relationship with epilepsy in adult MMD patients. It is suggested that some risk factors could be intervened to reduce the incidence of post-operative epilepsy in MMD patients.

Sitagliptin therapy improves myocardial perfusion and arteriolar collateralization in chronically ischemic myocardium: A pilot study
In conclusion, in chronically ischemic myocardium, sitagliptin improves myocardial perfusion and arteriolar collateralization via the activation of pro-arteriogenic signaling pathways.

Sex Difference in Capillary Reperfusion After Transient Middle Cerebral Artery Occlusion in Diabetic Mice
CONCLUSIONS: DM2 impairs capillary perfusion and exacerbates ischemic deep brain injury in male but not female young adult mice. Premenopausal females appear to be protected against DM2-related capillary dysfunction and brain injury.PMID:36689578 | DOI:10.1161/STROKEAHA.122.040972

E-142 Robotic transcranial doppler with artificial intelligence to identify cerebral emboli during transcatheter aortic valve replacement - a novel neuromonitoring tool
ConclusionIn this study, we report our experience using a Robotic Transcranial Doppler (TCD) with Artificial Intelligence as a novel real-time intraoperative neuromonitoring tool in patients undergoing TAVR. By reporting quantity and distribution of high intensity transient signals (HITS) as markers of microemboli during the procedure, we report significantly lower HITS using sentinel cerebral protection. We also identify valve positioning and implantation as the most significant stage in terms of HITS during the procedure.Abstract E-142 Table 1Emboli HITS during critical phases of the procedure Procedure Stage With Cere...

Posterior Circulation Ischaemic Stroke
Am J Med Sci. 2022 Jan 29:S0002-9629(22)00030-1. doi: 10.1016/j.amjms.2021.10.027. Online ahead of print.ABSTRACTPosterior circulation ischaemic stroke (PCIS) is a disease of high burden. They account for 20-25% of all ischaemic strokes. However, it is relatively under-researched and requires more clinical attention, since it carries worse functional outcomes. Vertigo, visual disturbances and sensory/motor disturbances are found in PCIS. Large artery atherosclerosis and embolism are main causes of PCIS, while there is growing evidence that vertebrobasilar dolichoectasia is a key association. Hypertension is the commonest r...

Clinical Relevance of Computed Tomography Perfusion-Estimated Infarct Volume in Acute Ischemic Stroke Patients within the 6-h Therapeutic Time Window
Conclusion: Larger initial infarct volumes are significantly associated with worse clinical outcomes in patients who underwent EVT because of early time window stroke. Furthermore, our study of 6-h data demonstrated that an initial infarction of more than a certain volume might be an independent risk factor for sICH development and poor outcomes even in patients with successful recanalization. However, we still observed benefits of EVT in patients with large ischemic cores. The CTP-estimated infarct volume might be an important prognostic factor after EVT rather than a biomarker predicting treatment effectiveness.Cerebrovasc Dis

Role of diabetes in collateral status assessed in CT perfusion –derived dynamic CTA in anterior circulation stroke
ConclusionThere was no statistically significant difference among patients with good, intermediate, and poor collaterals regarding the presence of diabetes or HbA1c level on admission. However, stroke patients with poor collaterals tend to have higher blood glucose and HbA1c levels.
