Cardiac Surgery in Indigenous Australians: Early Onset Cardiac Disease with follow-up Challenges
Conclusions: The mean age of 52 years at which Indigenous patients have cardiac surgery is significantly low compared to non-Indigenous patients. Indigenous patients have multiple risk factors for cardiac disease and with a large number requiring emergency surgery. Although surgical outcome in the short term is favourable, a large number of patients are lost to follow-up. The use of mechanical valve and warfarin should be individualised. Strategic post-operative follow-up mechanisms are needed to address these issues.

High survival rates in trial of new heart valve
Loyola University Medical Center is the only Chicago hospital participating in a landmark clinical trial of an artificial aortic heart valve that does not require open heart surgery. Frequency of and Determinants of Stroke After Surgical Aortic Valve Replacement in Patients With Previous Cardiac Surgery (from the Multicenter RECORD Initiative)
In conclusion, the risk of postoperative stroke after S-AVR in patients with previous cardiac surgery is high and has an impact on the immediate and late mortality. Excessive bleeding requiring blood transfusion and/or reexploration, prolonged cardiopulmonary bypass time, and use of intra-aortic balloon pump were associated with an extremely high rate of stroke.

Urgent surgery compared with fibrinolytic therapy for the treatment of left-sided prosthetic heart valve thrombosis: a systematic review and meta-analysis of observational studies
Conclusion Urgent surgery was not superior to FT at restoring valve function, but substantially reduced the occurrence of thrombo-embolic events, major bleeding, and recurrent PVT. In experienced centres, urgent surgery should probably be preferred over FT for treating left-sided PVT, pending the results of randomized controlled trials.

Abstract 227: Prescribing Warfarin at Discharge for Heart Failure Patients without Atrial Fibrillation Findings from the Get With The Guidelines-Heart Failure Registry Poster Session II
Conclusions: Approximately 1 in 10 HF patients without AF or other documented indications are prescribed warfarin at discharge, and prescription rates vary widely across hospitals. Further research is needed to understand why anticoagulation is prescribed to patients with HF in the absence of documented indications.
