Implementation of best medical therapy for cardiovascular risk factors in vascular surgery patients treated in a tertiary referral regional unit
ConclusionDespite increased implementation of best medical therapy in the community with compliance rates greater than 73% for aspirin and statin therapy, further improvement is warranted. Vascular surgeons should remain vigilant for further opportunities to optimise medical therapy in this high‐risk patient group particularly with antithrombotic, lipid lowering and antihypertensive therapies.

Outcomes of open surgical repair for chronic type B aortic dissections
Objective: Open surgical repair (OSR) for chronic type B aortic dissection (CTBAD) has an associated morbidity and mortality. The role of thoracic endovascular aortic repair (TEVAR) in CTBAD has not been determined. Parallel Endografts in the Treatment of Distal Aortic and Common Iliac Aneurysms.
CONCLUSIONS: Successful exclusion of D-AAA and BCIAA was achieved in high-risk patients using parallel endografts, allowing antegrade blood flow to one IIA when needed. Endovascular treatment for ruptured abdominal aortic aneurysm.
CONCLUSIONS: The conclusions of this review are currently limited by the paucity of data. From the data available there is no difference in the outcomes evaluated in this review between eEVAR and open repair, specifically 30-day mortality. Not enough information was provided for complications in order to make a well informed conclusion at this time. Long-term data are lacking for both survival and late complications. Upper extremity access for fenestrated endovascular aortic aneurysm repair is not associated with increased morbidity
Fenestrated endovascular aortic aneurysm repair (FEVAR) is an alternative to open repair in patients with complex abdominal aortic aneurysms who are neither fit nor suitable for standard open or endovascular repair. Chimney and snorkel grafts are other endovascular alternatives but frequently require bilateral upper extremity access that has been associated with a 3% to 10% risk of stroke. However, upper extremity access is also frequently required for FEVAR because of the caudal orientation of the visceral vessels.

Outcomes of Open Distal Aortic Aneurysm Repair in Patients With Chronic DeBakey Type I Dissection
We analyzed data regarding 198 open descending thoracic or thoracoabdominal aortic repairs in patients with chronic DeBakey type I aortic dissection. There were 14 early deaths (7%). Two patients (1%) developed permanent paraplegia, 5 (3%) had stroke, and 9 (5%) had renal failure. Actuarial 8-year survival was 65.6±5.9%.

Early and late mortality and morbidity of elective infrarenal aortic aneurysm repair.
CONCLUSION: The early and late mortality was similar after open and endovascular AAA repair. Postoperative complications did not show significant difference between the two groups. We found significant difference in the use of blood products, the prevalence of incisional hernias and the number of reinterventions. According to our results, stent graft implantation is mainly recommended in high risk patients and open aortic repair still has a role in the low-moderate operative risk group.
