Continuous Tunneled Infraclavicular Nerve Catheter for Palliative Management of Critical Limb Ischemia: A Case Report
We describe the successful use of a continuous local anesthetic infusion via an infraclavicular nerve catheter to control severe refractory ischemic upper limb pain in a patient with metastatic lung cancer for whom surgical and pharmacological intervention was unsuccessful. As her opioid requirements increased due to worsening ischemic pain, she subsequently developed opioid toxicity, hence prompting the palliative use of a tunneled infraclavicular nerve catheter under ultrasound guidance to minimize opioid requirements. We Care: A Wellness Intervention Project for Palliative Care Physicians
There is growing concern about the burnout rate among physicians in the United States, particularly amongst palliative care physicians. Kamal, et al. found the burnout rate to be 38.7% in 2020 among palliative care physicians (1). Reddy, et al. found that amongst palliative care physicians enrolled in a continuing education course, the burnout rate was also 38% (2). When asked to answer, “I feel burned out from my work,” 24% of the participants answered with a four or higher on the six-point Likert scale.

Opinion statementOpioid-induced neurotoxicity (OINT) is a neuropsychiatric syndrome observed with opioid therapy. The mechanism of OINT is thought to be multifactorial, and many risk factors may facilitate its development. If symptoms of OINT are seen, the prescriber should consider hydration, discontinuation of the offending opioid drug, or switching of opioid medication, or the use of some adjuvants.
