A case diagnoed ACOS with omelizumab treatment
ACOS Asthma- COPD overlap syndrome causes major problems in differential diagnosis of especially in smoking and older patients. ACOS carrying some characteristics of both asthma and COPD, is clinically markedly worst then both. Patients with ACOS, has a rapid worsening of pulmonary function tests, high ratios of exacerbations, multi symptoms, worst quality of life and a higher cost of administration. Estimated ratio of ACOS among asthmatic and COPD is as high as 15-55 %. In the history of 57 years old dyspneic patient there are childhood onset asthma, allergic rhinitis lasting about 20 years, diabetes mellitus and hyperten...

Pulmonary function changes following allogeneic haematopoietic cell transplantation
This study sought to identify factors associated with RFT decline post allogeneic HSCT in our cohort.Spirometry data from routine follow-up was collected from patients undergoing HSCT between Jan 2005 - Dec 2013 and survived to 1 year without relapse. Post HSCT FEV1 declines of >10% and subsequent recovery to within 10% of baseline were compared with pre-transplant factors: age, gender, smoking history, indication, pre-transplant RFT, cell source and use of anti-thymocyte globulin (ATG) during conditioning. Kaplan-Meier estimates were used to compare associations with FEV1 decline and recovery, Cox regression was used t...

Asthma phenotypes in low income children: A cluster analysis
Conclusion: Childhood asthma in low income Brazilian children is characterized by the presence of atopy, a high rate of exacerbation and fairly preserved lung function. Many similarities with the previous clusters indicate that this approach has good generalizability.

Effect of FESS on asthma patients with nasal polyposis and chronic sinusitis
Conclusion: Uncontrolled asthma associated with nasal polyp and chronic rinosinusitis can benefit from FESS in terms of pulmonary function test, asthma control level, nasal blockage symptoms and other sinonasal complaints.

Effect of vitamin d therapy on the pulmonary functions in pediatric asthma and allergic rhinitis
CONCLUSION: The study highlighted 61(92.5%) children had abnormal Vitamin D levels. The case group showed marked improvement in the lung functions. Adherence to the duration of therapy for a period of 10 weeks with Vitamin D is mandatory.

Bronchodilator responsiveness after ICS treatment
Conclusion: BDR test widely used for initial diagnosis, could be also a reliable tool in follow up children with asthma. In children with continuously positive BDR despite good ACQ results a step-up therapy change evaluation probably would be necessary.

Diagnostic Potential of Extracellular MicroRNA in Respiratory Diseases
AbstractLack of markers of subclinical disease state and clinical phenotype other than pulmonary function test has made the diagnosis and interventions of environmental respiratory diseases a major challenge. MicroRNAs (miRNAs), small non-coding single stranded RNAs, have emerged as potential disease-modifier in various environmental respiratory diseases. They can also be found in various body fluids and are remarkably stable. Because of their high stability, disease-specific expression, and the ease to detect and quantify them have raised the potential of miRNAs in body fluids to be useful clinical diagnostic biomarkers f...

Exercise-induced bronchoconstriction update —2016
The first practice parameter on exercise-induced bronchoconstriction (EIB) was published in 2010. This updated practice parameter was prepared 5  years later. In the ensuing years, there has been increased understanding of the pathogenesis of EIB and improved diagnosis of this disorder by using objective testing. At the time of this publication, observations included the following: dry powder mannitol for inhalation as a bronchial provocati on test is FDA approved however not currently available in the United States; if baseline pulmonary function test results are normal to near normal (before and after bronchodilator) in...

Systemic Reactions to Dust Mite Subcutaneous Immunotherapy: A 3-Year Follow-up Study.
CONCLUSIONS: The incidence of SRs to dust mite SCIT was low, and multiple factors were associated with the increased incidence of SRs. Children, asthmatics and patients with concomitant LR may be prone to develop SRs. PMID: 27334780 [PubMed]

Effect of sitting vs standing posture on spirometry in children
Asthma is a common medical condition that affects adults and children. National asthma guidelines (National Institutes of Health) recommend spirometry, a pulmonary function test, as an essential part of the initial diagnosis and follow-up care of asthma. Per the American Thoracic Society, traditionally spirometry is performed with the patient in the sitting posture, but usually it is not documented on the spirometry report. During the past 25 years, the rates of overweight and obesity have increased by 30% to 400% among 2- to 19-year-old.
