Real-life Cretan asthma registry focused on severe asthma: On behalf of 'The Cretan registry of the use of Biologics in Severe Asthma'
Exp Ther Med. 2021 Nov;22(5):1239. doi: 10.3892/etm.2021.10674. Epub 2021 Sep 1.ABSTRACTAsthma diagnosis and management remains a challenging task for the medical community. The aim of the present study was to present the functional and inflammatory profiles of patients with difficult-to-treat asthma in a real-life clinical setting referred to the specialized asthma clinic at the University Hospital of Heraklion. The registry included a cohort of 267 patients who were referred to the severe asthma clinic. Patients were assessed with emphasis on the history of allergies, nasal polyposis or other comorbidities. Blood testing...

Safety and efficacy of different antibiotic regimens in patients with ocular toxoplasmosis: systematic review and meta-analysis
ConclusionsNo antibiotic scheme was superior to others, and the selection of a treatment regimen depends on multiple factors; therefore, treatment should be chosen based on safety, sulfa allergies, and availability.

A 75-Year-Old Woman With Pulmonary Nodules and Dyspnea
Chest. 2021 Jul;160(1):e51-e56. doi: 10.1016/j.chest.2021.02.022.ABSTRACTA 75-year-old woman was referred to the pulmonary office in January 2020 for cough and progressive worsening of shortness of breath over the years. Her medical history was significant for asthma that was diagnosed approximately 10 years earlier, when she first developed dyspnea. A pre-bronchodilator spirometry at that time showed severe airflow obstruction (Fig 1). The patient was incidentally found to have several noncalcified pulmonary nodules on a chest CT scan in 2015, which was obtained as a part of dyspnea workup. She underwent bronchoscopic eva...

Antibiotics for asthma attacks: masking uncertainty
The role for antibiotics in acute asthma has been historically overestimated [1]. From a mechanistic point of view, multiplex PCR testing and conventional microbiological techniques show that >50–80% of events are associated with viral infections, and less than 20% associated with evidence of bacterial infection, with the remaining proportion presumed to be due to allergies and irritants [2]. Consequently, antibiotics are not expected to work in the context of most asthma attacks and their routine use is not recommended. This stance is supported by a Cochrane review, which found inconsistent data to support antimi...

What Are Some Indications for Using Dexamethasone?
Discussion Corticosteroids are a group of drugs which can be naturally or synthetically produced. Naturally occurring substances are produced in the adrenal gland, and are protein-bound (primarily corticosteroid-binding globulin and albumin). In the target tissues, they may need to be converted to an active substance. They are then reduced, oxidized, hydroxylated or conjugated as measures to inactivate them. Synthetic steroids have less protein binding and depending on their structure are more or less resistant to inactivation. Prednisone is the glucocorticoid most often used for treatment, especially as it has a short hal...

Medical care and treatment of children with asthmatic or wheezing health outcomes and urban-rural differences in Bavaria - a cross-sectional study
Conclusions: Prevalence rates of the diagnostic groups decreased compared to the HMU 2014/15. According to previous studies, factors associated with asthmatic or wheezing health outcomes could be confirmed. Children in rural areas generally received more medical care.PMID:33998939 | DOI:10.1080/02770903.2021.1926487

Successful Montelukast Treatment in an Infant with Steroid-Resistant Eosinophilic Colitis
We report an infant with steroid-resistant EC, who successfully returned to a healthy growth trajectory under the combined therapy of montelukast and ketotifen. An 8-month-old boy presented with bloody diarrhea, anemia, and failure to thrive (FTT) that started 6 days after birth. The patient has no known allergies. A trial of elementary diet was unsuccessful. The results of several stool cultures were unremarkable. Similarly, lower gastrointestinal series failed to identify anything significant. At 3 months of age, an esophagogastroduodenoscopy with biopsies from the distal duodenum and proximal jejunum were unremarkable. ...

Eosinophilic cystitis mimicking bladder cancer —considerations on the management based upon a case report and a review of the literature
AbstractThe hypereosinophilic syndrome (HES) is a rare disorder characterized by hypereosinophilia and infiltration of various organs with eosinophils. Eosinophilic cystitis (EC), mimicking bladder cancer clinically but also in ultrasound and in radiographic imaging, is one potential manifestation of the HES occurring in adults as well as in children. This case report describes the course of disease in a 57-year-old male presenting with severe gait disorders and symptoms of a low compliance bladder caused by a large retropubic tumor. After extensive urine and serologic examination and histologic confirmation of EC the pati...

Outdoor Allergies are Associated with Increased Oral Corticosteroid Use in Children with Allergic Asthma in New York City: A Single-Center Retrospective Study
Sensitization and exposure to environmental allergens is well known to be related to asthma morbidity; however, individual populations may vary with respect to specific risk factors associated with exacerbations. Among children with asthma in our urban community practices, we sought to better characterize children with the allergic asthma phenotype to identify allergic predictors of poor asthma outcomes.

Topical Corticosteroids and Glucocorticoid-Induced Osteoporosis —Dose and Duration Matter
Glucocorticoids are used by an estimated 0.5% to 1.0% of adults for management of a range of conditions, from allergies and inflammatory diseases to cancer. Glucocorticoid use increases with age: more than 3% of adults older than 50 years and 5.2% older than 80 years use glucocorticoids. Despite the availability of a growing number of medications to modulate immune function and/or inflammation, evidence from the Danish National Patient Registry suggests that the prevalence of glucocorticoid use has remained remarkably stable for 17 years (1999-2016). Thus, any noteworthy adverse effects associated with glucocorticoid use c...

The Relation of Allergy to Adenoid Hypertrophy and Otitis Media with Effusion: A Cross-sectional Study
According to this study, atopy was relatively common in patients with AHT and OME. Treatment with nasal corticosteroid and antihistamines were effective in these patients.PMID:33463120 | DOI:10.18502/ijaai.v19i5.4469
