The effect and associated factors of dispatcher recognition of stroke: A retrospective observational study.
CONCLUSIONS: The dispatchers should spend more time identifying stroke patients by following the dispatch protocol. Recognition of stroke by dispatchers was associated with improved stroke care. PMID: 29158105 [PubMed - as supplied by publisher]

Consensus Statements by Korean Society of Interventional Neuroradiology and Korean Stroke Society: Hyperacute Endovascular Treatment Workflow to Reduce Door-to-Reperfusion Time.
Abstract Recent clinical trials demonstrated the clinical benefit of endovascular treatment (EVT) in patients with acute ischemic stroke due to large vessel occlusion. These trials confirmed that good outcome after EVT depends on the time interval from symptom onset to reperfusion and that in-hospital delay leads to poor clinical outcome. However, there has been no universally accepted in-hospital workflow and performance benchmark for rapid reperfusion. Additionally, wide variety in workflow for EVT is present between each stroke centers. In this consensus statement, Korean Society of Interventional Neuroradiolog...

Age and gender differences in acute stroke hospital patients.
In conclusion, considerable differences were established between age and gender stroke patient groups, confirming the need of permanent national stroke registry and subsequent targeted action in secondary care, and prevention with education on risk factors, preferably personally tailored. Cardiovascular diseases in patients 65 years and younger with non-cardiogenic stroke.
CONCLUSIONS: Aetiologically heterogeneous stroke and cardiogenic stroke are the most commonly observed among young stroke patients. Cardiomyopathy and atrial fibrillation are the most common sources of cerebral embolism in young patients with cardiogenic stroke. Nearly 1/5 of patients with a non-cardiogenic stroke have congenital or acquired structural changes in the heart. PMID: 27279848 [PubMed]

Pre-hospital stroke screening and notification of patients with reperfusion-eligible acute ischaemic stroke using modified Face Arm Speech Time test.
CONCLUSION: Implementation of pre-hospital stroke screening using criteria based on a modified version of the FAST test, together with pre-arrival notification, significantly shortened the door-to-reperfusion therapy time for patients with ischaemic stroke. Pre-hospital stroke screening during ambulance transport by EMS personnel who complete a 2-hour focused training session is effective for identifying reperfusion-eligible patients with stroke. Predictors of early infection in cerebral ischemic stroke.
CONCLUSION: Patients with older age, tube feeding, lower conscious level, worse baseline stroke severity, large cerebral infarcts in CT scan, and increased IL-10 serum level were more susceptible to infection. The baseline serum level of IL-10 ≥ 14.5 pg/ ml and the size of infarct area > 3.5 cm3 were the independent predictors of PSI. PMID: 27453748 [PubMed - in process]

Improving acute stroke care in regional hospitals: clinical evaluation of the Victorian Stroke Telemedicine program.
CONCLUSIONS: Telemedicine has provided Victorian regional hospitals access to expert care for emergency department patients with suspected acute stroke. Eligible patients with ischaemic stroke are now receiving stroke thrombolysis more quickly and safely. PMID: 32255520 [PubMed - as supplied by publisher]

Outcome after intravenous thrombolysis in embolic stroke of undetermined source compared to cardioembolic stroke.
CONCLUSION: Patients with ESUS had better outcome after IVT than patients with CS, which can be attributed to younger age and milder strokes in these patients. PMID: 32876739 [PubMed - as supplied by publisher]

Monocyte count at onset predicts post ‐stroke outcomes during a 90‐day follow up
ConclusionsSmoker had more severe AIS and higher leukocytes and adhesion molecule levels. In the overall cohort, monocyte count was an independent predictor of worse post‐stroke outcome. Although larger trials are needed, monocyte count might be a cheap prognostic parameter in AIS.This article is protected by copyright. All rights reserved.

Recanalisation therapies for wake-up stroke.
CONCLUSIONS: There is insufficient evidence from randomised controlled trials for recommendations concerning recanalisation therapies for wake-up stroke. Results from ongoing trials will hopefully establish the efficacy and safety of such therapies. PMID: 30129656 [PubMed - as supplied by publisher]

Is there association between hyperdense middle cerebral artery sign on CT scan and time from stroke onset within the first 24-hours?
Conclusions: No evidence of a correlation between time of stroke onset and presence of a HAS within the first 24-h post acute ischaemic stroke was identified. The HAS was associated with a higher NIHSS score at presentation.

In light of recently published clinical trials and their implication for clinical practice, does a large catchment area acute hospital require 24  hour CT neck and head angiography and/or neuro-interventional services in the setting of acute ischaemic stroke?
ConclusionUHL should provide routine CT angiography to all patients presenting with acute ischaemic stroke in line with current guidelines. The need for provision of neuro-interventional services on-site proved more difficult to assess and requires further analysis.
