Alopecia areata is characterized by expansion of circulating Th2/Tc2/Th22, within the skin ‐homing and systemic T‐cell populations
ConclusionsAA is accompanied by Th2/Tc2 activation in skin homing and systemic subsets, correlating with disease severity, while IFN‐γ is linked to disease chronicity. These data hint for a possible role of diverse T‐cells subsets in disease pathogenesis, and emphasize the systemic nature of AA supporting the need for systemic therapeutic strategies in severe patients. This article is protected by copyright. Stevens Johnson Syndrome versus Jarisch Herxheimer Reaction in an HIV positive patient with secondary syphilis.
CONCLUSIONS: although the diagnosis of SJS has established criteria, the possibility of overlap with infectious illness and/or with its treatment, may complicate the diagnosis. PMID: 29076438 [PubMed - as supplied by publisher]

Effects of Orally Ingested Arsenic on Respiratory Epithelial Permeability to Bacteria and Small Molecules in Mice
Conclusions: Peroral arsenic has little effect on local airway immune responses to bacteria but compromises respiratory epithelial barrier integrity, increasing systemic translocation of inhaled pathogens and small molecules. https://doi.org/10.1289/EHP1878 Received: 09 March 2017 Revised: 14 August 2017 Accepted: 16 August 2017 Published: 28 September 2017 Address correspondence to M.B. Eosinophilic esophagitis: current evidence-based diagnosis and treatment in children and adults.
Authors: Lucendo AJ Abstract Eosinophilic esophagitis (EoE) is a disease caused by an immune response to food antigens in contact with the esophageal mucosa that has arisen as a common disorder in current clinical practice. Its diagnosis is defined by the combination of symptoms of esophageal dysfunction and inflammation of the esophageal mucosa predominantly by eosinophils. Its chronic course and frequent progression to subepithelial fibrosis leading to strictures and narrow-caliber esophagus indicate the need for treatment. Information provided by recent clinical trials and systematic reviews allowed for the deve...

Acute cardiac disease in a patient with hyper-IgE syndrome.
We describe the case of a 24-year-old male with hyper-IgE syndrome (HIES) which was diagnosed at 4 years of age and died from a very rare cardiac complication. He had typical clinical and laboratory manifestations of HIES, including total serum IgE as high as > 100,000 IU/mL. Stem cell transplantation was not available. During the 20-year follow-up, he suffered numerous various infections of the skin and deep organs, partial lung resection, as well as multiple bone fractures. At age 24, he developed acute decompensated heart failure associated with elevated serum troponin I and brain natriuretic protein. Two-dimensional...

124 Desmoplakin mutation in neonatal erythroderma
A neonatal boy presented with ichthyosiform erythroderma, hypotrichosis, recurrent sepsis and skin infections, failure to thrive, constant vomiting and brain abnormalities. He had persistent thrombocytosis, hypoalbunemia and hypernatremia. IgE levels rose at age 0.6 years (985 kU/l) without eosinophilia. Cow milk allergy was diagnosed at 0.7 years. IVIG treatment from 0.6 years reduced infections and acitretin from age 0.7 years reduced scaling. Skin biopsy showed a psoriformic reaction with normal LEKTI but absent filaggrin staining.

Histopathology of Urticaria
Opinion statementPurpose of reviewUrticaria is a common skin manifestation with many different causes. It is characterized by local or generalized edematous erythema with itching and increased blood flow and vascular permeability. Individual urticaria lesions typically last less than 24  h (usually less than 12 h), but new lesions may appear and resolve sporadically. Since the diagnosis is usually made based on the history and clinical appearance of the lesions, skin biopsy is not necessarily performed, but it is recommended for evaluating patients with refractory eruptions or su spicion of other diseases exhibiting urti...

Are metals involved in tattoo ‐related hypersensitivity reactions? A case report
ConclusionThe patient's hypersensitivity reaction and its spontaneous remission after removal of the implants indicate that Ti, possibly along with some of the other metals detected, could have played a major role in this particular case of tattoo‐related allergy.

Phase 1 healthy volunteer willingness to participate and enrollment preferences.
CONCLUSION: Our results suggest that healthy volunteers are willing to participate in a wide range of types of phase 1 clinical trials, and express preferences for low risk and familiar studies and study procedures, preferences which are partially affected by offers of payment. An imported case of leprosy in a trainee from Indonesia to Japan
A 33-year-old Indonesian man who was visiting Japan as a trainee in agricultural sector presented to the outpatient department of dermatology with a 1-year history of progressive rash. The rash had originated on his trunk and had gradually spread to his chest, abdomen, back and arms (Figure 1). The patient was not on any medications. He also denied any history of an allergy. Polymorphic, confluent, partially raised, hypopigmented macules that had progressed slowly were observed. The lesions demonstrated associated scaling, alopecia, and anesthesia. Although a left greater auricular nerve enlargement was detected, the patie...

The gene expression and immunohistochemical time ‐course of diphenylcyclopropenone‐induced contact allergy in healthy humans following repeated epicutaneous challenges
Abstract The gene expression time‐course of repeated challenge of contact allergy (CA) remains largely unknown. Therefore, using diphenylcyclopropenone (DPCP) as model allergen in healthy humans we set out to examine: (i) the monotonous and complex gene expression time‐course trajectories following repeated DPCP challenges to find the predominant gene expression pattern, (ii) the time‐course of cell infiltration following repeated DPCP challenges and (iii) the transcriptome of a repeated CA exposure model. We obtained punch biopsies from control and DPCP‐exposed skin from ten DPCP sensitized individuals at 5‐6 mo...

Effects of Coal Fly Ash Particulate Matter on the Antimicrobial Activity of Airway Surface Liquid
Conclusion: We propose that when CFA arrives at the airway, it rapidly adsorbs AMPs and creates negative complexes, thereby decreasing the functional amount of AMPs capable of killing pathogens. These results provide a novel translational insight into an early mechanism for how ambient PM increases the susceptibility of the airways to bacterial infection. https://doi.org/10.1289/EHP876 Received: 27 July 2016 Revised: 30 December 2016 Accepted: 19 January 2017 Published: 05 July 2017 Address correspondence to A. Vitiligo: Focus on Clinical Aspects, Immunopathogenesis, and Therapy
AbstractVitiligo is an acquired chronic depigmenting disorder of the skin, with an estimated prevalence of 0.5% of the general population, characterized by the development of white macules resulting from a loss of epidermal melanocytes. The nomenclature has been revised after an extensive international work within the vitiligo global issues consensus conference, and vitiligo (formerly non-segmental vitiligo) is now a consensus umbrella term for all forms of generalized vitiligo. Two other subsets of vitiligo are segmental vitiligo and unclassified/undetermined vitiligo, which corresponds to focal disease and rare variants....

Nonlesional atopic dermatitis skin shares similar T ‐cell clones with lesional tissues
ConclusionWhile AD harbors a highly polyclonal T‐cell receptor repertoire, and despite the lack of information on TCR antigen specificity, the sharing of top abundant clones between lesional and nonlesional skin, and their persistence after months of therapy, points to the continuous presence of potentially pathogenic skin resident memory T cells well beyond clinically inflamed lesions.
