Atrial fibrillation and stroke - Coexistence and attitude to preventive therapy on the basis of Szczecin and Szczecin region patients.
The objective of the study was to evaluate the frequency of coexistence of non-valvular AF and IS during the acute stroke and to analyze the attitude of AF patients to treatment. The study included 3712 successive patients presenting either an IS or a transient ischemic attack. The analysis revealed a significant increase in the rate of patients with AF and IS in the years 2010-2013 (31.9%) compared with 2002-2005 (20.2%). A rise in the proportion of AF and IS patients was recorded over the course of consecutive years in group II. The proportion of newly detected AF cases during hospital stay differed significantly between...

Pathways to Pulmonary Hypertension In Sickle Cell Disease: The Search For Prevention And Early Intervention.
Authors: Shilo NR, Morris CR Abstract INTRODUCTION: Pulmonary hypertension (PH) develops in a significant number of patients with sickle cell disease (SCD), resulting in increased morbidity and mortality. This review focuses on PH pathophysiology, risk stratification, and new recommendations for screening and treatment for patients with SCD. Areas covered: An extensive PubMed literature search was performed. While the pathophysiology of PH in SCD is yet to be fully deciphered, it is known that the etiology is multifactorial; hemolysis, hypercoagulability, hypoxemia, ischemic-reperfusion injury, oxidative stress, an...

Universal HIV Testing and Treatment (UTT) Integrated with Chronic Disease Screening and Treatment: the SEARCH study
AbstractPurpose of ReviewThe growing burden of untreated chronic disease among persons with HIV (PWH) threatens to reverse heath gains from ART expansion. Universal test and treat (UTT) ’s population-based approach provides opportunity to jointly identify and treat HIV and other chronic diseases. This review’s purpose is to describe SEARCH UTT study’s integrated disease strategy and related approaches in Sub-Saharan Africa.Recent FindingsIn SEARCH, 97% of adults were HIV tested, 85% were screened for hypertension, and 79% for diabetes at health fairs after 2  years, for an additional $1.16/person. After 3 years, po...

Comparison of the Effect of Combination Therapy with an Angiotensin II Receptor Blocker and Either a Low-Dose Diuretic or Calcium Channel Blocker on Cardiac Hypertrophy in Patients with Hypertension.
Abstract Left ventricular hypertrophy (LVH) regression is an important issue in hypertensive patients. Patients with LVH who had received the angiotensin receptor blocker (ARB) treatment for 8 weeks and had not reached the target blood pressure level were enrolled in the study. Patients were assigned to either losartan (50 mg)/hydrochlorothiazide (HCTZ, 12.5 mg) group or ARB + CCB group (usual dose of ARB and calcium channel blocker, CCB). After 48 weeks, LV mass index was found to be reduced significantly in the losartan/HCTZ group but not in the ARB + CCB group. These results suggest that combination thera...

The Elusive Search for Optimal Blood Pressure Targets
AbstractBP treatment thresholds/targets determine when to initiate treatment and to what level BP should be reduced. The Seventh Joint National Committee on Prevention, Detection, Evaluation, and Treatment of High Blood Pressure (JNC-7) recommended a target of<140/90 for most patients and a target<130/80  mmHg for patients with diabetes or chronic kidney disease. Subsequently, meta-analyses, retrospective studies relating on-treatment BP to clinical outcomes and two large, randomized clinical trials (RCTs) have re-evaluated BP targets. In Action to Control Cardiovascular Risk in Diabetes (ACCORD), a systolic blood p...

High rates of viral suppression in adults and children with high CD4+ counts using a streamlined ART delivery model in the SEARCH trial in rural Uganda and Kenya.
CONCLUSIONS: Using a streamlined care model, viral suppression, retention and ART safety were high among asymptomatic East African adults and children with high CD4+ counts initiating treatment. CLINICAL TRIAL NUMBER: NCT01864603. PMID: 28770596 [PubMed - in process]

Cerebral amyloid angiopathy-related cognitive impairment: The search for a specific neuropsychological pattern.
Authors: Planton M, Raposo N, Albucher JF, Pariente J Abstract Cerebral amyloid angiopathy is diagnosed in stroke units after lobar intracerebral hemorrhage. CAA can also be diagnosed in memory clinics when patients are referred for cognitive impairment assessment, and may be a reason for admission to emergency or neurology departments because of rapidly progressive cognitive or neurological decline, or a transient focal neurological episode. CAA may even be observed in older community-dwelling individuals. Neuropsychological impairment in CAA has been described over the past 20 years. The symptoms most commonly re...

Cardiovascular autonomic neuropathy in adolescents and young adults with type 1 and type 2 diabetes: The SEARCH for Diabetes in Youth Cohort Study
ConclusionsThe prevalence of CAN in this multiethnic cohort of adolescents and young adults with type 1 and type 2 diabetes are comparable to those reported in adults with diabetes. Suboptimal glycemic control and elevated triglycerides were the modifiable risk factors associated with CAN.

Search of multiple markers of organ damage for better cardiovascular risk stratification in hypertension: Role of “SHATS” syndrome in the clinical practice
Essential hypertension is a chronic, asymptomatic disease characterized by sustained and persistent rise in systolic/diastolic blood pressure (BP) levels and increased risk of developing major cardiovascular (CV) complications, including stroke, myocardial infarction, renal disease, congestive heart failure and CV death. Risk of hypertension-related CV complications can be estimated by comprehensive evaluation of individual global CV risk profile, which includes search for CV risk factors and comorbidities, as well as assessment of markers of organ damage (OD).

The incessant search for renal biomarkers: is it really justified?
Purpose of review This review summarizes the most recent and relevant findings in the search for novel biomarkers for the most common renal diseases. Recent findings Unprecedented, fast-paced technical advances in biomedical research have offered an opportunity to identify novel and more specific renal biomarkers in several clinical settings. However, despite the huge efforts made, the molecules identified so far have generally failed to provide relevant information beyond what has already been generated by established biomarkers, such as serum creatinine and proteinuria, whereas the complexity and costs of these tech...

Genetics Of Atrial Fibrilation: In Search Of Novel Therapeutic Targets.
Authors: Lozano-Velasco E, Garcia-Padilla C, Aránega AE, Franco D Abstract Atrial fibrillation (AF) is the most frequent arrhythmogenic disease in humans, ranging from 2% in the general population and raising up to 10-12% in 80+ years. Genetic analyses of AF familiar cases have identified a series of point mutations in distinct ion channels, supporting a causative link. However, these genetic defects only explain a minority of AF patients. Genome-wide association studies identified single nucleotide polymorphisms (SNPs), close to PITX2 on 4q25 chromosome, that are highly associated to AF. Subsequent GWAS studies h...
