Utility-Weighted Modified Rankin Scale as Primary Outcome in Stroke Trials Clinical Sciences
Conclusions—The UW-mRS as primary outcome does not capture individual variation in utility values and may reduce the statistical power of a randomized trial.

Time to Endovascular Treatment and Outcome in Acute Ischemic Stroke: MR CLEAN Registry Results.
Conclusions -Time to EVT for acute ischemic stroke in current clinical practice is strongly associated with functional outcome. Our data suggest that this association might be even stronger than previously suggested in reports on more selected patient populations from randomized controlled trials. These findings emphasize that functional outcome of EVT patients can be greatly improved by shortening onset to treatment times. PMID: 29581124 [PubMed - as supplied by publisher]

Short versus long duration of dual antiplatelet therapy following drug-eluting stents: a  meta-analysis of randomised trials.
CONCLUSIONS: Short DAPT following DES implantation results in a significant reduction of major bleeding events with no apparent increase in all-cause death, myocardial infarction, stent thrombosis, or stroke. Future dedicated trials should investigate the optimal strategies for patient-tailored DAPT in various subgroups. PMID: 29541996 [PubMed - as supplied by publisher]

Associations Between Collateral Status and Thrombus Characteristics and Their Impact in Anterior Circulation Stroke Clinical Sciences
Conclusions—Patients with lower thrombus burden and higher thrombus perviousness scores had higher collateral score. The positive effect of thrombus perviousness on clinical outcome was only present in patients with moderate and high collateral scores.Clinical Trial Registration—URL: http://www.trialregister.nl. Familial hypercholesterolaemia in patients with ischaemic stroke or transient ischaemic attack
ConclusionsThe frequency of potential FH, based on clinical criteria, in patients with acute ischaemic stroke or TIA was 11.5% and that of probable/definite FH (1.3%) was similar to recently reported counts for patients with acute coronary syndrome (1.6%). FH screening using the DLCNA is feasible in clinical routine and should be considered as part of the usual diagnostic work‐up.

Long-term Fine Particulate Matter Exposure and Nonaccidental and Cause-specific Mortality in a Large National Cohort of Chinese Men
Conclusions: Long-term exposure to PM2.5 was associated with nonaccidental, CVD, lung cancer, and COPD mortality in China. The IER estimator may underestimate the excess relative risk of cause-specific mortality due to long-term exposure to PM2.5 over the exposure range experienced in China and other low- and middle-income countries. https://doi.org/10.1289/EHP1673 Received: 24 February 2017 Revised: 01 September 2017 Accepted: 05 September 2017 Published: 07 November 2017 Address correspondence to M. Longer Ischemic Time is Associated with Increased Ventricular Stiffness as Measured by Pressure –Volume Loop Analysis in Pediatric Heart Transplant Recipients
ConclusionsOur study found that longer ischemic times are associated with increased left ventricular stiffness. No other single clinical variable is associated with cardiac dysfunction as determined by PVL analysis.

Techniques for endovascular treatment of acute ischemic stroke.
Authors: Pierot L, Gawlitza M, Soize S Abstract Early recanalization of occluded vessels in patients with acute ischemic stroke (AIS) by either intravenous thrombolysis (IVT) or endovascular revascularization has been shown to be associated with improved clinical outcomes and reduced mortality. Since the initial report regarding endovascular treatment (EVT) of AIS in 1983, endovascular techniques have been tremendously improved, advancing from intra-arterial administration of thrombolytic drugs to stent retrievers. IVT has been evaluated in several large randomized trials and has been shown to improve clinical outc...

Insertable Cardiac Monitor Versus Standard of Care For Detection Of Atrial Fibrillation In Patients Following Cryptogenic Stroke: A Dutch Cost-Effectiveness Analysis
Documentation of atrial fibrillation (AF) is required to initiate oral anticoagulation therapy for recurrent stroke prevention. The cause of ischemic stroke remains uncertain despite a complete diagnostic evaluation in 20-40% of cases (cryptogenic stroke), and conventional standard of care (SoC) often fails to diagnose AF. An insertable cardiac monitor (ICM) is a diagnostic device which has been shown to improve detection of AF in this patient population. We evaluated the cost-effectiveness of ICM versus SoC, in patients with a cryptogenic stroke, from a Dutch payer perspective.

Value of Thrombus CT Characteristics in Patients with Acute Ischemic Stroke ADULT BRAIN
CONCLUSIONS: In our study on patients with large-vessel occlusion of the anterior circulation, CT thrombus characteristics appear useful for predicting functional outcome. However, in our study cohort, the effect of intra-arterial treatment was independent of the thrombus CT characteristics. Therefore, no arguments were provided to select patients for intra-arterial treatment using thrombus CT characteristics.
