'Dying of heartache?' Heart problems linked to bereavement
Conclusion The study found that people were more likely to have AF for the first time in the weeks immediately after a bereavement, but that this raised risk does not last. Despite the headlines, this does not mean that anyone who's had a bereavement is at immediate risk of "dying of a broken heart". Atrial fibrillation is treatable and not usually life-threatening. Because this was an observational study, we cannot rule out the possibility that factors such as family history of atrial fibrillation or lifestyle factors could have affected the results, although the researcher's conclusion that this is a small poss...

Palinopsia in a Patient with an Acute Occipital Lobe Stroke (P4.369)
Discussion: Palinopsia is the persistence or recurrence of visual images after the visual stimulus has been removed. Palinopsia can be further subtyped with one of the categories being scene perseveration, where a formed scene may continuously replay for a length of time either superimposed in a visual field or in place of an impaired visual field. There have been rare other cases described with right hemispheric strokes causing similar phenomena of scene perseveration. Our case is unique that in previously reported literature, it was very rare to have isolated palinopsia without hemianopsia (1 out of 32 patients in one st...

Diabetes Mellitus and Prevention of Late Myocardial Infarction After Coronary Stenting in the Randomized Dual Antiplatelet Therapy Study.
CONCLUSIONS: -In patients with DM, continued thienopyridine beyond 1-year after coronary stenting is associated with reduced risk of MI, although this benefit is attenuated when compared with patients without DM. Concomitant Use of Single Antiplatelet Therapy With Edoxaban or Warfarin in Patients With Atrial Fibrillation: Analysis From the ENGAGE AF-TIMI48 Trial Arrhythmia and Electrophysiology
Conclusions Patients with AF who were selected by their physicians to receive SAPT in addition to an anticoagulant had a similar risk of stroke/SEE and higher rates of bleeding than those not receiving SAPT. Edoxaban exhibited similar relative efficacy and reduced bleeding compared to warfarin, with or without concomitant SAPT. Clinical Trial Registration URL: http://www.clinicaltrials.gov/. Unique identifier: NCT00781391.

Vorapaxar in the secondary prevention of atherothrombosis.
Authors: Tantry US, Liu F, Chen G, Gurbel PA Abstract Dual antiplatelet therapy with aspirin, a platelet cyclooxygenase-1 inhibitor and P2Y12 receptor blockers, remains the major drug strategy to prevent ischemic event occurrence in patients with acute coronary syndromes and in patients undergoing coronary stenting, but there some limitations that can be overcome by targeting novel targets. Unlike direct thrombin inhibitors that bind directly to thrombin, targeting the platelet thrombin receptor, protease activated receptor (PAR)-1, may offer a better choice for the attenuation of atherosclerosis progression, throm...

Pulse Pressure and Risk for Cardiovascular Events in Patients With Atherothrombosis From the REACH Registry
BackgroundPulse pressure (PP) provides valuable prognostic information in specific populations, but few studies have assessed its value on cardiovascular outcomes in a broad, worldwide population.ObjectivesThe aim of this study was to determine whether PP is associated with major adverse cardiovascular outcomes, independently of mean arterial pressure.MethodsParticipants from the international REACH (Reduction of Atherothrombosis for Continued Health) registry, which evaluates subjects with clinical atherothrombotic disease or risk factors for its development, were examined. Those with incomplete 4-year follow-up or PP dat...

STEMI vs NSTEACS management trends in non-invasive hospital
Conclusion NSTEACS patients in western province of KSA present at an older age are mostly males and have higher prevalence of hypertension and hyperlipidemia compared with STEMI patients. It is therefore important to identify patients with high-risk profile and put implement measures to reduce these factors.

Stroke subtypes and interventional studies for transient ischemic attack.
Authors: Lavallée P, Amarenco P Abstract Transient ischemic attack (TIA) is the most important risk factor for ischemic stroke. The risk is the highest in the first hours after symptom onset, and treatment must be initiated in emergency. In the acute phase, antithrombotic agent is probably the most important treatment, but it is not excluded that lipid-lowering agents and/or antihypertensive drugs are also important. For current guidelines, monotherapy of antiplatelet agent remains the gold standard in emergency. However, most recent data and meta-analysis support a combination therapy of clopidogrel and aspirin. ...

The first prognostic model for stroke and death in patients with systolic heart failure.
CONCLUSIONS: This novel prognostic model predicts the overall risk of ischemic stroke or death for HF patients. This model compares favorably for death with the Seattle model and has the added utility of including stroke as an endpoint. Use of this model will help identify those patients in need of more intensive monitoring and therapy and may help identify appropriate populations for trials of new therapies. My Approach to Using Coronary Artery Calcium Scoring in Primary Prevention of Atherosclerotic Cardiovascular Disease
Cardiology guidelines universally recommend consideration of preventive pharmacotherapy, including aspirin and statin therapy, based on estimations of absolute atherosclerotic cardiovascular disease (ASCVD) risk using various risk estimators. These tools use established ASCVD risk factors such as age, gender, blood pressure, cholesterol levels, diabetes, and smoking. The most recent risk score is the centerpiece of the 2013 ACC/AHA prevention guidelines, and it estimates 10-year risk for a heart attack or stroke.
