Inappropriate use of proton pump inhibitor among elderly patients in British Columbia: what are the long-term adverse events?
CONCLUSION: This study revealed a high prevalence of PPI use among elderly populations in BC, Canada. The overutilization of PPIs is often a result of failure to re-evaluate the need for continuation of therapy. Published studies identified signals of serious harm from long-term PPI exposure. Healthcare providers with patients can reverse the relentless expansion of long-term PPI exposure by discussing the expected benefits and potential harms.PMID:37496243 | DOI:10.2174/1574886318666230726124540

Chemotherapy and Targeted Therapy Strategies in Patients with Unresectable or Borderline Resectable Metastatic Colorectal Cancer: Evidence for a Lack of Focus on Resection Rates
CONCLUSION: The ORR and survival rates in unresectable and borderline resectable mCRC were improved by modern CTx and TT that did not translate into higher RR, mostly due to the lack of expert involvement.PMID:37644249 | DOI:10.1245/s10434-023-14049-3

Multi-marker Solid Tumor Panels Using Next-generation Sequencing to Direct Molecularly Targeted Therapies
Conclusions Recent advances in sequencing technology provide new opportunities for genomic medicine. With these opportunities come the promises of personalized medicine that are changing oncology practice. However, aspects of analytic and clinical validity and clinical utility of the current paradigm shift has yet to be clearly established. Developing the necessary evidence to establish clinical validity and utility may require novel thinking to adapt to the dynamic challenges associated with implementing NGS tumor sequencing into clinical practice. Researchers must also address several limitations in the underlying concep...

Treatment strategies for colorectal carcinoma with synchronous liver metastases: Which way to go?
CONCLUSION: None of the aforementioned strategies appears inferior. It is necessary to establish individual treatment plans in multidisciplinary team meetings through careful appraisal of all strategies. PMID: 26078580 [PubMed - in process]

Incidence and treatment of recurrent disease after cytoreductive surgery and intraperitoneal chemotherapy for peritoneally metastasized colorectal cancer: a systematic review
Conclusion Recurrence is very common after cytoreductive surgery and intraperitoneal chemotherapy for PC of colorectal origin. Repeat cytoreductive surgery suggests a potential survival benefit for a highly selected group. Therefore, strategies to prevent recurrence are of the utmost importance.

This study is the first to show that downregulation of PAPP-A expression in adult mice can significantly extend life span. Importantly, this beneficial longevity phenotype is distinct from the dwarfism of long-lived PAPP-A KO, Ames dwarf, Snell dwarf and growth hormone receptor (GHR) KO mice with germ-line mutations. Thus, downregulation of PAPP-A expression joins other treatment regimens, such as resveratrol, rapamycin and dietary restriction, which can extend life span when started in mice as adults. In a recent study, inducible knockdown of the GHR in young adult female mice increased maximal, but not median, lif...

Locating sex- and gender-specific data in health promotion research: evaluating the sensitivity and precision of published filters.
CONCLUSIONS: Although search filters can facilitate the identification of research evidence to enable decision making, variability in study abstracting and indexing can limit the generalizability and usability of these filters. This potential for variability should be considered when deciding to incorporate a search filter into any literature search. This research highlights the importance of this awareness when developing strategies for searching the published literature and the potential value of supplementing database searching with other methods of study identification. Moving Beyond Conventional Clinical Trial End Points in Treatment-Refractory Metastatic Colorectal Cancer: A Composite Quality-of-Life and Symptom Control End Point.
Abstract PURPOSE: This review highlights the evidence supporting symptom control and quality-of-life (QOL) measures as predictors of survival in treatment-refractory metastatic colorectal cancer (mCRC) and describes a composite symptom control and QOL end point recently reported in a Phase III trial that may serve as a more reasonable end point of efficacy in this population. METHODS: A literature search was conducted using MEDLINE to identify clinical studies (including case series and observational, retrospective, and prospective studies) that reported the predictive value of QOL measures for survival in mC...

Clinical trials in menopause
Conclusions:The available literature suggests that HT is a viable option for the primary prevention of cardiovascular disease in postmenopausal women. Newer trials will likely verify this assessment. If this is enough to change clinical practice, however, remains to be seen given the general fear of HT by many with prescriptive authority, and also the women in our care. Objective: Clinical trials in menopause have undergone much scrutiny over the years. This has led to significant shifts in the treatment of symptomatic menopause and a substantial impact on women. We aim to delineate the key studies contributing to this...

Hematological Side Effects of Immune Checkpoint Inhibitors: The Example of Immune-Related Thrombocytopenia
Conclusion Hem-irAEs are potentially life-threatening complications of ICIs that require a high level of clinical suspicion. Ir-TCP is amongst the most frequently reported hem-irAEs. Due to its low incidence and the lack of specific diagnostic criteria, ir-TCP may be underdiagnosed and possibly underreported. The diagnosis is based on exclusion of other causes of TCP together with clinical features that suggest an immune event such as the refractoriness to platelet transfusions and response to corticosteroids. During clinical development, a systematic analysis of ir-TCP cases at clinical study and program level is of outm...

Digesting the Contents: an Analysis of Online Colorectal Cancer Education Websites
This study aims to provide the most in-depth and comprehensive examination of online colorectal resources to date. An internet search using the terms “colorectal cancer,” “colon cancer,” and “rectal cancer” were performed, with meta-search engines “Dogpile,” “Yippy,” and “Google.” A total of 741, 759, and 703 websites were returned by the search engines respectively. A list of the “top 100 websites” was compiled by pla cing the sites in average rank order. The websites were evaluated using a validated structured rating tool. Inter-rater reliability was evaluated using kappa statistics, and the r...

Fight Aging! Newsletter, November 9th 2020
In this study, young adult mice were submitted to endurance exercise training and the function, differentiation, and metabolic characteristics of satellite cells were investigated in vivo and in vitro. We found that injured muscles from endurance-exercised mice display improved regenerative capacity, demonstrated through higher densities of newly formed myofibres compared with controls (evidenced by an increase in embryonic myosin heavy chain expression), as well as lower inflammation (evidenced by quantifying CD68-marked macrophages), and reduced fibrosis. Enhanced myogenic function was accompanied by an increased ...
