Visceral Leishmaniasis Masquerading as Drug-Induced Pancytopenia in Lung Cancer Patients
Curr Oncol. 2024 Apr 17;31(4):2274-2277. doi: 10.3390/curroncol31040168.ABSTRACTMaintenance chemotherapy is a standard treatment in patients with non-progressive advance staged IV non-squamous non-small cell lung cancer after induction therapy. Here, we report the case of a 53-year-old man undergoing a maintenance monotherapy with pemetrexed who presented prolonged pancytopenia despite filgrastim injections. A bone marrow aspiration revealed a macrophage activation syndrome with Leishmania amastigotes. A Polymerase Chest Reaction testing confirmed the diagnosis of visceral leishmaniasis. Treatment with liposomal amphoteric...

Barking up the wrong tree: Vascular tree-in-bud due to intravascular lymphoma
Respir Med Case Rep. 2024 Apr 5;49:102020. doi: 10.1016/j.rmcr.2024.102020. eCollection 2024.ABSTRACTA 59-year-old previously healthy woman presented with a six-month history of fever, nonproductive cough, and weight loss. The cause of these symptoms remained obscure despite a thorough, month-long hospitalization. On presentation, she was normotensive with a pulse of 98 beats/minute, respiratory rate of 20 breaths/minute, and a temperature of 39.4C. She was emaciated. Physical examination was notable for faint bibasilar crackles on lung auscultation. Initial laboratory testing revealed pancytopenia. Peripheral smear demons...

A young patient with Graves' disease presenting with a triad of heart failure, pancytopenia, and jaundice: A case report
We describe a case of a young man who presented with progressive cough, increased abdominal girth and ankle swelling for a few months. On examination, he had jaundice, bilateral exophthalmos, diffuse goiter, ascites, and significant lower limb edema. Laboratory investigations showed increased Thyroxin level with a suppressed thyroid stimulating hormone and positive anti-thyrotropin receptor antibodies. Also, the patient had pancytopenia, coagulopathy and cholestatic pattern of elevated liver enzymes. Echocardiography demonstrated mildly reduced left ventricular function with diastolic dysfunction, but electrocardiogram did...

Sore throat, fever, and pancytopenia during winter
Key Clinical MessageCare must be taken to mitigate the effect of cognitive bias in times of frequent common presentations. The etiology of bicytopenias and pancytopenias must always be carefully investigated. Blast cells in low count B ALL may not be seen on a peripheral smear and diagnosis often requires confirmational bone marrow aspirate with flow cytometry and molecular typing. Spontaneous clearance of serum HCV-RNA after splenectomy in a patient with HCV-related liver cirrhosis and portal hypertension: a case report
CONCLUSIONS: We have presented a case of spontaneous clearance of HCV-RNA after splenectomy in a patient with cirrhosis and portal hypertension. Splenectomy may be associated with disappearance of HCV-RNA based on previous reports. More cases should be accumulated and evaluated.PMID:38647617 | DOI:10.1186/s40792-024-01899-6 (Source: Hand Surgery)

Triclonal Gammopathy in Multiple Myeloma with Bleeding Diathesis: A Case Report
ConclusionIn plasma cell dyscrasias and lymphoproliferative diseases, monoclonal gammopathies are often observed, while the biclonal variety is less frequent. However, tri clonal gammopathy having a combination of IgM Kappa, IgG Kappa, and IgA Lambda is extremely rare and their clinical significance is unknown. Nephronophthisis 13 caused by WDR19 variants with pancytopenia: case report
We present a case of nephronophthisis 13 that resulted fromWDR19 variants. The patient, a nine-year-old Japanese boy, had detection of mild proteinuria during a school urine screening. Urinalysis revealed mild proteinuria without hematuria. Blood tests indicated pancytopenia, mild elevation of liver enzymes, and kidney dysfunction. Ultrasound examination disclosed hepatosplenomegaly. Abdominal computed tomography and bone marrow assessments ruled out malignant tumors. Subsequent kidney and liver biopsies suggested nephronophthisis and congenital hepatic fibrosis. Furthermore, comprehensive genetic analysis through next-gen...

Missed opportunities for HIV testing and sexual health-related challenges in an individual with intellectual disability: a case report
CONCLUSION: Due to the presence of existing indicator conditions, an earlier HIV diagnosis was possible. We contend that most of the recent illnesses could have been prevented if earlier testing had been carried out. Therefore, patients presenting with AIDS indicator conditions, including those with mental disabilities, should be given the opportunity to be tested for HIV. HIV/AIDS trainings should be made available to health care professionals as well as to personnel interacting with vulnerable groups.PMID:38581028 | DOI:10.1186/s12981-024-00606-7 (Source: Cell Research)
