Jaundice with severe leucocytosis
A 36-year-old Hispanic woman was admitted to the emergency department with jaundice, abdominal pain and fatigue. The patient reported daily alcohol consumption (70 units per week). Physical examination revealed jaundice, hepatosplenomegaly, high BMI (30 kg/m2) and significant bilateral oedema of the lower limbs. Complete blood count showed a severe leucocytosis with neutrophilia (white blood cell count at 48,000/ μl including 85% neutrophils) associated with bicytopenia (haemoglobin at 4.3 g/dl and platelet count at 87,000/μl). The detrimental health consequences of alcohol use disorders are undisputed, but the impact of political actions to address this on a societal level has been unclear. D íaz, Fuentes-López and coworkers analysed alcohol-related public health policies (PHPs) in 169 countries, from which they developed an alcohol preparedness index (API). The API quantified differences in alcohol-related PHPs, and countries with a higher API had reduced prevalence of alcohol use dis orders, as well as reduced mortality due to alcohol-related liver diseases, cancers and cardiovascular disease. Use of stem cell-derived hepatocytes to model liver disease
Liver disease is a leading cause of global mortality and a multibillion-dollar per year economic burden that presents an urgent target for research.1 Established model systems have proven useful in advancing the field. However, constraints exist which have impeded the translatability of scientific findings derived from these systems. Human stem cell-derived hepatocytes have emerged as a highly relevant technology that can be used to model liver disease with increased availability, lower cost, and more faithful replication of hepatic physiology than traditional model systems. Reply to: “Hepatic encephalopathy and survival after transjugular intra-hepatic portosystemic shunt: Do spontaneous portosystemic shunts matter?”
Overt hepatic encephalopathy (OHE) may negatively impact the quality of life of patients and their caregivers, and our study aimed to investigate whether episodic OHE following a transjugular intrahepatic portosystemic shunt (TIPS) procedure adversely affects survival rates. Our findings suggest that episodic post-derivative OHE does not affect patient survival. On the contrary, we found that the development of persistent OHE leads to worse survival rates, particularly in patients with more advanced liver disease. (Source: Journal of Hepatology)

Exploring the impact of the PNPLA3 I148M variant on primary human hepatic stellate cells using 3D extracellular matrix models
