Changing from NAFLD to MASLD: Similar Prognosis of Unresectable Extrahepatic Gastrointestinal Cancer Under Chemotherapy between NAFLD and MASLD
The multisociety consensus group has redefined the nomenclature of nonalcoholic fatty liver disease (NAFLD) to metabolic dysfunction-associated steatotic liver disease (MASLD) in 2023.1 This revision enabled us to better capture the underlying etiologic pathways of the disease and eliminate stigmatization potential.1,2 As definitional modifications often yield considerable confusion from numerous perspectives, it is likely that we will encounter obstacles for some time. In particular, in order to avoid wasting valuable research resources, it is most important to extend previous research on NAFLD to MASLD, and several repor...

A new clinical and prognostic characterization of the patterns of decompensation of cirrhosis
The bad prognosis of Acute Decompensation (AD), i.e. the development of complications that require hospitalization, has recently been assessed. However, complications of cirrhosis do not necessarily require hospitalization and can develop progressively, as in the recently defined Non-Acute Decompensation (NAD). Nevertheless, there is no data regarding the incidence and prognostic impact of NAD. The aim of the study was to evaluate the incidence and the prognostic impact of NAD and AD in outpatients with cirrhosis. (Source: Journal of Hepatology)

Uric acid may serve as the sixth cardiometabolic criterion for defining MASLD
The definition of MASLD (metabolic dysfunction-associated steatotic liver disease) incorporates five cardiometabolic criteria that emphasize the impact of metabolic disorders 1. These criteria revolve around hypertension, hyperglycemia, and hyperlipidemia. However, we realize that hyperuricemia (HUA), known as the "fourth hyper" 2, is not currently included. In today's society, the incidence of hyperuricemia is rapidly increasing due to high-purine diets and sugary beverages 3, and the correlation between HUA and fatty liver has been reported 4. Battle of the Biopsies: Role of tissue and liquid biopsy in hepatocellular carcinoma
Hepatocellular carcinoma (HCC) diagnosis and management have undergone significant improvements in recent years. With the introduction of immunotherapy-based combination therapy, there has been a notable expansion in treatment options for patients with unresectable HCC. Simultaneously, innovative molecular tests for early detection and management of HCC are emerging. This progress prompts a key question: as liquid biopsy techniques rise in prominence, will they replace traditional tissue biopsies, or will both techniques remain relevant? Given the ongoing challenges of early HCC detection, including issues with ultrasound ...

An open-label randomised-controlled trial of azathioprine vs. mycophenolate mofetil for the induction of remission in treatment-naive autoimmune hepatitis
Patients with autoimmune hepatitis (AIH) almost inevitably require lifelong immunosuppressive treatment. There is genuine concern about the efficacy and tolerability of the current standard combination therapy of prednisolone and azathioprine. Mycophenolate mofetil (MMF) has emerged as an alternative option. The aim of this study was to compare MMF to azathioprine as induction therapy. (Source: Journal of Hepatology)

Long-term use of tenofovir disoproxil fumarate increases fracture risk in elderly patients with chronic hepatitis B
The use of tenofovir disoproxil fumarate (TDF) is associated with a reduction in bone mineral density and an increase in bone metabolism biomarkers. However, data on clinical bone fractures remain limited. We evaluated the impact of TDF compared to entecavir on the risk of fracture in elderly patients with chronic hepatitis B (CHB). Reply to: “5-MTHF enhances the portal pressure reduction achieved with propranolol in patients with cirrhosis: A randomized placebo-controlled trial. Was an accurate statistical analysis performed?”
We thank Dr Fernandez-Garza for his interest in our work and for his comments on the statistical analysis. He underlines that our intention-to-treat analysis (ITT) did not include all planned randomized patients (30 per group) and that we do not explain why.1 We would like to clarify this point.
